Job summary

The Haringey and Islington Locality Teams are integrated teams consisting of specialist community rehabilitation therapists and district nurses that work together in an integrated way to support the long-term health needs of adult Haringey and Islington patients.

The Locality services will be available to support an individual's nursing and rehabilitation needs. They provide a person-centred approach to optimise independence and confidence, enable recovery and prevent a decline in functional ability.

The Rehabilitation Clinical Lead will clinically lead and manage the general rehabilitation teams across Haringey and Islington. It's a role that is expected to modernise and transform pathways. This role is underpinned by clinical practice, area specific clinical competence, leadership and management and education.

Job responsibilities

The Haringey and Islington Locality Teams will be integrated teams consisting of specialist community rehabilitation therapists and district nurses that work together in an integrated way to support the long-term health needs of adult Haringey and Islington patients.

The Locality services will be available to support an individuals nursing and rehabilitation needs. They provide a person-centred approach to optimise independence and confidence, enable recovery and prevent a decline in functional ability.

This will: enable people to live healthy independent lives for as long as possible in their own homes, or the place they call home reduce the need for escalation of care to non-home settings facilitate timely return to their usual place of residence following temporary escalations of care to non-home settings

Its a role that is expected to modernise and transform pathways. This role is underpinned by clinical practice, area specific clinical competence, leadership and management and post holder will clinically lead and manage the general rehabilitation teams across Haringey and Islington. Provide highly specialised rehabilitation assessments for a range of complex patients within the local health economy. Provide expert clinical advice and support to clinical staff working in rehabilitation services. Ensure that patients are managed through the rehabilitation pathway. Contribute to the development of rehabilitation pathways for patients referred to the team. To participate in clinical audits and quality improvement projects. To ensure the development, provision and maintenance of high-quality services are commensurate with the mission and aims of Whittington Health To ensure the delivery of high-quality care and customer services, continual service improvement, service re-design and development, managing staff performance, and providing professional advice on service improvements. To work with colleagues within and outside of the ACS ICSU in the development and redesign of services. To lead on the continuing development of Haringey and Islington rehab areas in the light of current health strategies and improve integrated working with other agencies. This will include ensuring clinical standards of practice, advising or leading on recruitment and retention of staff in these teams, and giving professional advice to other service managers To be an effective change agent, bringing about service development by proactively seeking out new methods of working, initiating changes in policy or procedures, and reviewing the skill mix within the teams. To coordinate and ensure the delivery of ongoing training for all staff members and to ensure equity and opportunity of access to training. To foster cross-professional working with professionals across the Trust in order to improve patient delivery and care, including developing training, liaison and joint working.

MAIN RESPONSIBILITIES Clinical To be professionally and legally accountable for all aspects of own work, ensuring a high standard of clinical care for patients under own management and supporting other clinicians to do likewise. To practice without direct supervision as an autonomous practitioner according to professional framework and scope of practice To assess patients with a wide range of highly complex conditions, utilising expert clinical reasoning, problem solving skill, evidence-based knowledge anda wide range of assessment skills to formulate accurate prognoses and recommend the best course of intervention. To assess the rehabilitation needs of patients and to recommend the most appropriate rehabilitation service based on this assessment. To be an expert clinical resource to peers and all members of the multidisciplinary team within the Locality Team. To maintain, continue to develop and update own expert practice through continuous professional development. To provide evidence to support this in the form of a personal portfolio. To identify and access appropriate sources of clinical support, establishing and maintaining networks across professional and organisational boundaries. To ensure own Personal Development plan is in line with the Trust and service objectives. To provide highly specialist therapy to a clinical caseload of patients requiring community-based rehabilitation. To articulate and convey highly complex clinical findings, results of investigations and their implications for management, with patients and colleagues in Primary, Secondary and Tertiary Care. To articulate effectively the specialist and expert therapy perspective on a patients condition with members of the multidisciplinary team and negotiate when various patient management options are available. To maintain and develop partnerships with others to improve patient care and service delivery. To clearly convey highly complex knowledge of anatomy, pathophysiology (including neurology, sensory and cognitive aspects) and to be able to describe a variety of assessment and treatment models to patients, their relatives and staff. To clearly communicate assessment, progress and treatment results to patients, referrers, commissioners and others, both verbally and in writing. This includes writing reports for legal and statutory purposes. To promote good working relationships and a positive team climate. To keep the manager informed of relevant research findings and of any implications for the Rehabilitation service. To initiate and participate in research, audit and quality improvement projects within the Trust. To participate in the Trust and service Clinical Governance Plan. To identify appropriate outcome measures and service impact measures that accurately evaluate patient response and service development need To keep abreast of national agendas and published frameworks, new evidence, guidelines, standards, protocols and policies both clinical and professional related to role. To use this knowledge to contribute to the development of the Rehabilitation service so that the services comply with these agendas, policies and guidelines.

Managing the Workforce Support the Locality Manager and Operational lead to manage the flow of patients through the services. To manage the rehabilitation team in close collaboration with the Locality Manager and Operational lead. To assist Whittington Health in the development of clinical pathways, protocols and other patient management tasks and to be responsible for their production and implementation when delegated to do so. To record own activity and collect other information as required and to present this in a timely fashion. Write reports as necessary for managers or commissioners , on the use of external rehabilitation facilities. To make recommendations on the most clinically effective use of resources. To contribute to business planning, service development and demand and capacity management, by providing timely advice and information to the Locality manager, Borough Leads and Commissioners of Adults and Older People services on all matters relating to the Rehabilitation pathway.

Strategy, Change Management and Service Improvement: To work with clinicians and managers and staff to implement the Trust Clinical Strategic Direction in order to continuously improve the quality and timeliness of services to enhance the patients experience, reduce waiting times and minimise inefficiencies. To be proactively involved in developing Care Pathways, either by offering expert professional advice or by leading on the development and implementation of amended processes or practices within community teams under your management. This may involve developing new roles for staff and addressing the clinical governance and development needs associated with this. To cascade corporate strategic ideas and information to all staff within the teams managed whilst being sensitive to their levels of understanding and prior knowledge. To identify both new opportunities for the individual services within the service, arising from changes in the external environment or internal innovation and potential threats and ways to respond to them. Contribute to the development and delivery of the ICSU strategy and policies taking lead responsibility for operational delivery within Specialist services. Ensure that the service supports the delivering of the requirements of the national NHS agenda, activity and capacity requirements are available to support business plans, and that all compliance policies are followed.

Working in Partnership To ensure effective communication and involvement arrangements exist across partner agencies at borough and cross-borough level for Locality services. To represent the organisation with external agencies including the local authority, other service providers, patients, lead clinicians, GPs, CCGs, community groups and other appropriate agencies on issues relating to service delivery and development

General To work across sites where Adult Community Services are delivered as required. To undertake other relevant duties as requested.

Education / Qualifications

Essential

Degree qualification in physiotherapy /OT/SLT Current registration with HCPC

Desirable

MSc qualification in Advanced clinical practice or willingness to work towards it

Skills

Essential

Able to communicate highly complex and sensitive information effectively in written, verbal and presentational form to patients, staff, GPs and other senior clinicians in an environment which challenges preconceptions about care of patients. Demonstrate the ability to understand and implement Clinical Governance, and its relationship to quality Demonstrate ability to justify clinical decisions with Consultants, GPs, commissioners, all relevant staff across a range of providers, patients, relatives and carers and the general public if required. Clinical leadership skills, including development of evidence-based practice guidelines and leading on service improvements. Evidence of experience supervising staff

Knowledge

Essential

Extensive experience as a clinical therapist, including working in a Highly Specialist Community therapy role. Evidence of extensive post-graduate clinical training Demonstrate and understanding of approaches to rehabilitation in an acute, rehabilitation and community environment. Demonstrate the ability to develop expert knowledge in devising and implementing a management programme for total patient care. Understanding of issues involved in offering a service to a diverse community, and of practical ways to ensure that the service is culturally appropriate and accessible.
