An opportunity has become available for a Band6 Occupational Therapist to join our expanding community team. The role supports stroke patients that havebeen discharged home from hospital. This role will contribute to us rolling outthe ICSS model. The job would be part or full time over 7 dayson a rota system covering the county of Worcestershire. We support flexibleworking, and the Trust is excellent in supporting well-being.
As a trustwe offer a robust induction period and continue to promote opportunities fordevelopment through 1-1 supervision session, peer working and in-servicetraining.
Main duties of the job
Day to day, you will be undertaking comprehensiveassessments of mild-moderate-severe stroke patients including those withmultiple pathologies/co-morbidities, using specialist OT assessment tools toidentify their personalised goals alongside the MDT. You will formulateindividual programmes of rehabilitation using clinical reasoning and utilisinga range of treatment skills, targeting realistic and measurable, patient-led goals.It is also expected that you will offer support and education tocarers/families.
You will attend the weekly MDT in which the team willdiscuss the progress of patients and identify goals that are achieved andongoing. You will also participate in referral management which will includethe triaging of referrals, liaising with other health care professionals withinthe Trust/Acute Trust and surrounding borders.
We pride ourselves on being truly interdisciplinary, so youwill also be involved with supporting patients to undertake their personalisedrehab programmes to promote independence. These sessions are part of arehabilitation focused approach of which we can evidence great results inpatients regaining independence.
