The shift pattern is Monday-Friday 09.00-17.00.

An exciting opportunity has arisen for an Advanced Diabetes Nurse Specialist to work within the District Nursing Service within Renfrewshire HSCP.

The Advanced Diabetes Specialist Nurse works in partnership with the wider multi-disciplinary team and across the acute and community interface to deliver holistic, individualised programmes of care to patients with diabetes who are referred by either primary or secondary care.

The post holder will work in conjunction with the referrer to provide highly specialist advice in order to optimise diabetes symptom control. They are required to undertake highly skilled assessment and clinical interventions at various stages for individuals living with this chronic disease process.

The post holder will have comprehensive, highly specialist knowledge within the field of diabetes and act autonomously in the assessment, planning and implementation of treatment plans for patients with a diagnosis of diabetes. They will have overall responsibility for managing complex caseloads whilst delegating patients to the wider DN team.

The post holder will be responsible for the design and delivery of specialist diabetes education and support to adults with diabetes, their families and carers and the wider MDT team across the Primary and secondary care interface.

The post holder will act as a resource for expert advice and training for nursing staff, medical staff, other health care professionals and other agencies. The post holder will lead and contribute to issues relating to diabetes care locally and nationally where appropriate

The post holder is responsible for the delivery of the highest standards of patient centred care to patients and their relatives through the development and management of staff within their remit.

The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience and will lead and enable the team to provide a high quality service.
