Role Summary

This is an exciting opportunity to join a new team specialising in Frailty, Rehabilitation, Dementia and Elderly Care.

Fortescue ward is a bedded ward designed specifically to meet the needs of patients with dementia and rehabilitation needs. Our aim is to promote independence and improve the patient experience while in hospital. We work closely with our Therapy team to run daily exercise classes, as well as breakfast and lunch clubs. We aim to increase the activities on the ward so that patients can be assessed in a way that is not formal or planned. Our involvement with the National Frailty Network has developed our understanding of how important it is to get patients home as quickly as possible, this is a great opportunity for a band 6 Nurse to develop skills and leadership in complex discharge planning, often working alongside Mental Health teams, outside services and families to achieve the best outcome for the patient.

The Ward is part of the Health and Social Care project “Promoting Independence” with a focus on what is important to the patient. Involvement in the project presents an opportunity to integrate with teams outside the acute setting and explore the range of services available and be part of changes to services to keep people safe at home.

We are passionate about the learning and development of others. Candidates will be expected to have or be willing to undertake the mentorship qualification.

We are looking to recruit a Nurse into a band 6 role who wishes to develop skills in ward management and leadership but also has a passion for enhancing the care of elderly frail patients and those with dementia. This is a fantastic opportunity to develop within the organisation and support will be given to attend events to widen your experience, including deputising in the ward managers absence.

Some experience of managing the acutely unwell patient is also desirable as we firmly believe that patients with dementia should not be moved to other areas when in hospital and where possible they should remain on Fortescue ward.

Working hours per week including weekends and bank holidays
