Job summary

Anexciting opportunity has arisen for an appropriately skilled and experiencedregistered nurse to join Kirklees Health and Care Partership's ContinuingHealthcare (CHC Team) as a lead nurse for the learning disability and mentalhealth CHC Team. Although direct experience of working in a CHC team is notessential, candidates must be able to demonstrate knowledge and experience ofthe CHC legislation and process and have experience of working within eitherthe MH or LD field of practice.

Assessment,including risk assessment, and case management skills are essential.

Youshould have excellent communication and interpersonal skills with the abilityto produce clear and concise written reports.

Full-timeand part-time hours considered.

Main duties of the job

The post holder will be responsible forundertaking NHS Continuing Healthcare/ Funded Nursing Care assessments, careplanning and the evaluation of packages for patients eligible for NHSContinuing Healthcare. They will supportcomplex case management and work in collaboration with other healthprofessionals and social care staff. The post holder will deputise for the band7 nurses. The post holder will haveresponsibility for agreeing eligibility and will make financial decisionswithin the NHS Kirklees Health and Care Partnership standing financialinstructions.

Job responsibilities

Thepost holder will be responsible for undertaking NHS Continuing Healthcare/Funded Nursing Care assessments, care planning and the evaluation of packagesfor patients eligible for NHS Continuing Healthcare. The postholder is expectedto work independently within codes of practice, standard operating proceduresand professional guidelines.

Main Duties and Responsibilities

ThePost holder will support complex case management and work in collaboration withother health professionals and social care staff. The post holder will deputisefor the band 7 nurses.

Communications & Relationship Skills

UndertakingContinuing Healthcare (CHC)/Funded Nursing Care (FNC) assessments and reviewsfor patients across all aspects of care.This necessitates a high level of assessment skills requiring tact,diplomacy and the ability to liaise effectively with colleagues, health careprofessionals, service users' carers and family members.

Offeringcontinuing support and advice to family and carers in complex and sensitive situations,necessitating a high level of interpersonal, and reassurance skills; supportingthe local resolution process in the case of complaints/appeals/retrospectivecases and providing information on a timely basis.

Developingand maintaining effective and appropriate communication networks with healthand social care professionals and act as a resource to them.

Workingwith relevant health professionals, CHC colleagues, Business Support Staff,Senior Leads and other Support Staff within the CHC team to ensure a seamlessand efficient service is delivered.

Analytical & Judgmental Skills

Exercisingprofessional judgement as necessary when analysing and communicating complexand sensitive information on behalf of the CHC service and application of theassociated criteria.

Interpretingnational and regional guidance relating to the implementation of the CHCNational Framework and subsequent developments within the CHC service.

Decisionmaking with regards to multi-disciplinary team CHC assessments to determineeligibility.

SupportingCommunity Matrons/District Nurses in their case management roles for complexcases that are eligible for continuing healthcare.

Planning & Organisational Skills

Deputisefor the CHC Team Leader when required.

Deliveragainst organisational objectives, achieving quality outcomes, prioritising ownworkload and working to tight deadlines.

Plan, organise and implement reviewsand assessments, to co-ordinate with Local Authority, family and providers

Patient/Client Care

Ensuringthere is a holistic care plan in place, including the production and co productionwhen required with Social Services.

Ensurepersonal health budgets are actively promoted with service users and theirrepresentatives and work closely with the CSU to encourage a seamless delivery

Identify risk of abuse and need forprotection from harm to self or others and report in line with localsafeguarding routes and governance arrangements.

Ensuringtimely "End of life" provision and reviews, following the nationalguidance for clients Fast Tracked through the CHC system.

Providingspecialist nursing advice to secondary care, primary care, nursing homes etc tofacilitate the continuing healthcare process.

Policy and Service Development

Ensurepolicies are implemented and adhered to whilst identifying and proposingchanges for improvement to practices and procedures in own area. Required to implement changes to policies andprocedures.

Finance and Physical Resources

Approvinga package of care in line with the NHS Kirklees Health and Care Partnershipstanding financial instructions and agreed thresholds.

Continuallystrive for delivering project/function outcomes, value for money and greaterefficiency

Agreeingeligibility and will make financial decisions within the NHS Kirklees Healthand Care Partnership standing financial instructions

Human Resources

Deliveringformal and non formal education programmes to Community and hospital staff,including nursing/residential homes, GPs, community nursing and therapy staffrelating to the assessment and management of CHC patient groups in line withthe latest research, evidence and Department of Health/NICE guidance.

Information Resources

Maintainingand updating clinical records in a timely manner and in line with local andnational policies.

Educated to post-registration or graduate diploma level or equivalent level of training supported by experience. -Hold a Registered Nurse Qualification. -Current and relevant NMC registration
