As anOPAT/IV Nurse, you'll work alongside an expert team to provide cutting-edge,patient-centred care. This role offers you the chance to use your clinicalskills to support early hospital discharges, prevent unnecessary admissions,and deliver exceptional care to patients needing IV antibiotics or long-termvascular access management.

Beyondpatient care, youll collaborate with the OPAT team leaders to provideguidance, training, and advice to community nurses, secondary care teams, andGPs, making a lasting impact on healthcare delivery across the region.

Our service operates from 7am to 8pm, seven daysa week, offering a flexible and rewarding environment where your skills andpassion will shine. This is your opportunity to join an innovative teamdedicated to improving outcomes and reshaping community healthcare.

Main duties of the job

Your keyresponsibilities will include

Performing peripheral cannulation, venepuncture, and port-a-cathprocedures in line with established protocols.

Supporting district nurses by providing guidance and assessment in theadministration of intravenous antibiotics.

Assisting with the delivery of intravenous therapy in patients homes,ensuring a safe and effective process.

Providing clinical advice and support to patients and district nursingteams during periods when the IV Team Leader is unavailable, such as duringholidays or study leave.

Ensuring a smooth and seamless transition of care for patients byadopting a multi-disciplinary approach.

Delivering care in a clinic or patients home without directsupervision, while maintaining effective communication with the IV Team Leader.

Preparing detailed and accurate written updates on patients progressfor review by the IV Team Leader.

Assisting in the development, implementation, and evaluation of patientcare plans.

Supporting the education and skill development of community nurses toensure competency in intravenous therapy.

Working within Trust policies and guidelines and following intravenoustherapy patient care pathways.

Engaging with patients, families, and healthcare professionals throughclear and effective communication.

Job responsibilities

PrincipalResponsibilities

To perform port-a-cath proceduresaccording to established protocols.

To act as an assessor to districtnurses in the assessment of the administration of intravenous antibiotics.

To provide clinical support andadvice to the patient and district nurses in the absence of the IV Team Leader. during holidays, study leave.

To work alongside the district nurseteams to support them in providing intravenous therapy within the patientshome environment.

To work under the direction of theIV Team Leader.

To work within Trust policies andguidelines and follow the intravenous therapy patient care pathways.

To provide a safe, seamlesstransition of care by using a multi-disciplinary approach to care.

To work without direct supervisionof the IV Team Leader in the patients own environment and also within a clinicsetting. Reporting to the IV Team Leader.

To support the assessment,development, implementation and evaluation of care.

To provide safe discharge of patientcare from the pathway.

To provide accurate writtencommunication of the patients progress to IV Team Leader.

To support the training/skills ofcompetencies of other community nurses within intravenous antibiotic therapy.

Effectively communicate verballywith patients, their families and other health care professionals in the courseof their work.

Support clinical supervision andclinical governance within the team.

Maintain professional portfolio thatdemonstrates evidence of on-going professional development.

Ensure that practice isevidence-based through audit and critically appraising literature.

Maintain clinical standards throughreflective practice and clinical supervision.

Training

Act as amentor for pre registration nursing students.

To provide clinical education andtraining to patients and care givers in the administration of intravenousantibiotic therapy under protocol.

Support the development of thenursing team by teaching and assessing junior members of the team.

Undertake mandatory in-housetraining and other training that supports the role.

Teaching of other healthprofessionals to support patients in maintaining their optimum level ofindependence.
