About the Role

We are seeking a highly skilled Community Nurse to join our team in Warrington. As a key member of our District Nursing team, you will play a vital role in providing high-quality patient care and supporting patients to remain independent in their own homes.

Key Responsibilities

  • Promoting health and wellbeing within families and communities.
  • Performing comprehensive assessments of patient nursing needs as the named nurse.
  • Planning, assessing, and evaluating programs of care according to changing health needs under the direction of the District Nurse.
  • SUPPORTING THE TEAM LEADER IN THE CO-ORDINATION OF COMPLEX AND CONTINUING CARE PACKAGES.
  • Maintaining accurate records in accordance with NMC.
  • Attending multi-disciplinary case conferences as the named nurse if appropriate.
  • Communicating and collaborating with other members of the PHCT, voluntary, and statutory organisations to support individual patient care.
  • Establishing and maintaining effective communication with patients, relatives, and carers in complex and potentially challenging situations.
  • Competently performing clinical skills related to the post.
  • Being professionally responsible for a delegated caseload under the supervision of the District Nurse.
  • Assessing health needs at individual, practice, and community levels.
  • Undertaking chronic disease management reviews using appropriate templates.
  • Working without direct supervision.

About You

To be successful in this role, you will need:

  • A minimum of 6 months paid experience in a community nursing environment within a Community Trust in the last 3 years.
  • Experience with wound care, insulin and medication administration, catheter care, and palliative care.
  • NMC Professional Registration.
  • To comply with NMC Professional Registration and keep up to date with current nursing practices.
  • To ensure accurate records are kept in accordance with CHS policy and NMC guidelines.
  • To undertake regular mandatory training.
