About the Role Chelmer PCNhas an opportunity for a care co-ordinator to join our primary care network (PCN), to support our4GP practices across the Chelmsford area to assist in re-shaping primary care across our communities. This role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service. The main duties of the role include: Supporting the clinical directors in coordinating all key activity, including access to services, advice, and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred. Supporting digital initiatives. Responsibility for the co-ordination of the patients' journeys through primary care. About the Candidate

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups. Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification. Strong listening and communication skills. Familiar with local resources and services, including how to access them. Motivated by helping people, with care and empathy. Able to deliver person-centred support in a non-judgmental way. Experience of completing holistic person-centred care planning assessments. About Us

Our PCN includes the following surgeries: Chelmer Village Surgery, North Chelmsford Healthcare Centre, Rivermead Gate Medical Centre and Sutherland Lodge.
