Main duties of the job

You will play a vital role in our multidisciplinary services; managing patients with complex needs in the community.

You'll have the freedom to plan care in your own way, with time to develop your assessment and treatment skills, and access to structured supervision and a clinical lead to make sure you are received the support necessary.

You'll be supporting junior colleagues with clinical development, helping them to reach their potential and develop the workforce of the future. You will be required to drive between patient homes and work sites and will need to provide your own transport to achieve this.

Qualifications

Essential

Registered health care professional (NMC or HCPC registration) / minimum of 2 years of experience in health improvement environment Must have valid UK NMC PIN

Desirable

Evidence of Specialist qualifications relevant to area of practice (examples non-medical prescribing; mentorship, coaching, clinical expertise) Health related degree and evidence of relevant post graduate development at Level 6 PTLLS/DTLLS, PGCE or Certificate in Education Management or supervisory qualification or experience of study Working towards Master level study

Experience

Minimum of 2 years' experience in area of practice Practical experience in managing health related projects/complex caseloads Supporting others (mentor / educator / supervisor) experience
