Job description

Job responsibilities

To effectively co-ordinate the care of a complex caseload of service users with severe and enduring mental illness.

To undertake initial and detailed assessments where this is clinically appropriate.

To formulate care plans in line with identified need.

To provide recovery focussed therapeutic interventions in line with the agreed care plan and risk assessment.

To assess risk, develop risk management plans, review and adjust in line with the service user's needs.

To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

To use tools which promote user empowerment Wellness Recovery Action Plans (WRAP)

To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the post holder provides those interventions. This will include physical health monitoring.

To have a proactive approach to the role and needs of carers / families.

To be proactive in monitoring mental health needs and respond in a timely fashion if needs change adjusting the care plan accordingly.

To participate fully in caseload supervision and management, to ensure that care plans are matched to need.

To work with relevant agencies, procedures and legislative requirements, in particular, The NHS and Community Care Act, Mental Health Act, Data Protection Act, Adult & Child Protection Procedures.

To work in accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding

To deliver evidence based interventions in accordance with those agreed for use within the service

To actively monitor and review the effects of prescribed medication for the patient group noting its efficacy and the affect of potential side effects and acting promptly to escalate concerns or seek advice regarding potential medication changes.

To administer prescribed medication where required including depot medication in accordance with agreed Medicines Procedures

To develop a professional relationship with service users based on principles of recovery.

To comply with requirements to monitor and record agreed outcome measures and key performance indicators mental health minimum data set.

To accurately record all contacts with service users in the RiO electronic record in line with Southern Health FT record keeping policy.

To ensure effective communication with service users, relatives and carers, visitors, staff and others.

To ensure that all communication takes place in a manner that is consistent with legislation, policies and procedures.
