Join Our Team as a Specialist Community Forensic Team Practitioner

We are seeking a highly skilled and dedicated Specialist Community Forensic Team Practitioner to join our multi-disciplinary team. As a key member of our team, you will play a vital role in providing high-quality care to individuals with severe and enduring mental illness and a forensic history.

About the Role

As a Specialist Community Forensic Team Practitioner, you will be responsible for assessing, monitoring, and reviewing the social care needs of individuals, relatives, and carers within a specific care group. You will work closely with our multi-disciplinary team, including forensic psychiatry, mental health nursing, clinical psychology, social work, and occupational therapy, to provide comprehensive care and support.

Key Responsibilities
  • Act as care coordinator to an agreed caseload of service users
  • Use professional knowledge to assess, monitor, and review social care needs
  • Work collaboratively with the multi-disciplinary team to provide comprehensive care and support
