Job summary

The Long Term Conditions (LTC) andPopulation Health (PH) Programme Manager will be responsible foroperationalising plans for LTCs and population health management (PHM).

Working in partnership the LTC and PHProgramme Manager will support decision making and strategic planning acrossorganisational boundaries acting as a vehicle for improvement.

The LTC and PH Programme Manager will support and drive transformation as wellas securing value for money in planning, commissioning and service quality. Effectiveproject and change management expertise will be required to ensuretransformation of services to improve access to services for patients,relatives and carers. The post-holder will manage and implement the SNEE ICS PHM Programmewithin each Alliance, improving outcomes for the population.

The Strategic Programmes team works across SNEE ICBfootprint which includes Ipswich & East Suffolk, West Suffolk and NorthEast Essex alliances ensuring transformational developments are in place acrossthe Integrated Care System (ICS) footprint.

Please note, due to the ongoing cost reduction programme and restructure underway at SNEE ICB (due to end in Summer 2024), prior consideration for this role may be given to existing staff who are affected by this programme and this role may be removed from the recruitment process at any time.

Main duties of the job

SNEE ICB has a five-year joint forward plan the Long-TermConditions and Population Health Programme Manager will be responsible foroperationalising the plan for long term conditions and population healthmanagement. Effective project and changemanagement expertise will be required to ensure transformation of services toimprove access to services for patients, relatives and carers.

The Long-Term Conditions and Population Health Programme Manager will support local projects and drive transformation as well assecuring value for money in planning, commissioning, and service quality.

Manage andimplement the SNEE ICS Population Health Management Programme within eachAlliance, developing and embedding the use of linked data, predictive analyticsand partnership working in developing models of care and interventions toimprove outcomes for our most vulnerable groups and improve health outcomes forthe population.

Work with Analytical teams to analyse andinterrogate data for all programs of work; work with transformation andalliance leads to prioritise; work with health and care practitioners,multi-disciplinary teams, and voluntary sector workers to design and deliverthe interventions for PHM and LTC area.

Job responsibilities

TheStrategic Programmes team works across the Ipswich & East Suffolk, WestSuffolk and North East Essex alliances ensuring transformational developmentsare in place across the Integrated Care System (ICS) footprint. The Programme supports the delivery of NHSEngland National priorities in the long term plan, the SNEE Joint Forward planand aligns the approaches undertaken in each ICS Alliance. This approach enables a more patient-centredfocus to develop both providers and commissioner and their relationships withone another, across all care settings (community, primary, secondary, tertiarycare). Health outcomes will improvethrough helping commissioners reduce unwarranted variation in services andencouraging innovation to deliver effective change to health care servicesthrough greater collaboration between providers and commissioners acrossdifferent levels of the health care system.

The post holders may come from a variety of backgrounds, includingclinical or care professional, public health, improvement, transformation, oranalytics. Their strength will be in enabling sound communication and guidance,bridging the gap between analytical staff and practitioners to deliver theinterventions identified. The successfulcandidates will need to be experienced in using data, evidence and intelligenceto improve services. They will be ableto build relationships and translate PHM intelligence so staff can understandand apply methods and ideas, developing staff competencies in PHM.

Working inpartnership with commissioners (including local government) and providers the Long-TermConditions and Population Health Programme Manager will support decision makingand strategic planning across organisational boundaries acting as a vehicle forimprovement for patients, carers, commissioners, providers, voluntary organisations,and the public in order to:

Enable clinical and patientengagement: informing commissioning decisions

Define and drive qualityimprovement: operating across complex pathways of care

Coordinate and supportcommissioners and providers: identifying and reducing unwarranted variation,improving cohesion and ensuring sustainable services across pathways of carefor staff and patients, both now and in the future.

Knowledge, Training & Experience

Essential

Highly developed specialist knowledge across range of procedures underpinned by theory and experience Knowledge of a range of specific project areas, acquired through post graduate diploma or equivalent experience or training, plus further specialist project management knowledge or experience to masters level equivalent Educated to masters level or equivalent of experience of working at a senior level in specialist area Extensive knowledge of specialist area, acquired through post-graduate diploma or equivalent experience or training plus further specialist knowledge or experience to masters level equivalent. Evidence of post qualifying and continuing professional development Must understand the background to and aims of current healthcare policy and appreciate the implications of this on engagement
