Main duties of the job
With the use of Clinical Systems and engagement with Practice the care coordinator will actively identify and manage a caseload of patients who are in need of a care package. Work with a cohort of identified patients to coordinate and navigate necessary personalised care requirements in line with PCSP best practice. Process relevant requests from hospital discharge letters, support with palliative care, safeguarding and relevant referrals. Help patients to manage needs, answering questions, supporting to make appointments, signposting to supporting agencies, training, employment and benefits. Ensure that patients have good quality information to help them make informed choices about their care. Ensure excellent communication channels with the patient and wider care system regarding patient progress. Liaise with multi agencies to coordinate pathways of patient care. Ensure close working relationships with other PCN roles such as Social Prescribers, Health Coaches and Dieticians. Support Quality and Outcome Frameworks and Local and National Targets. To include a focus on DES Specifications, in particular supporting cancer and care home initiatives. Provide all round support to the rest of the administrative team (including Reception and Secretarial duties)

Job responsibilities
This role involves supporting patients through variousstages of treatment pathways. The Care Coordinator will work closely with GPsand other Primary Care professionals within the Practice to identify a caseloadof identified patients, ensuring that the appropriate support is made availableto them and their families.
Tackling health inequalities with a focus on localpriorities is pivotal as well as providing much needed advice and guidance to adiverse population of patients. Working within an MDT model of support, theCare Coordinator will develop in line with best practice to offer an excellentservice for vulnerable patients. An ethos of promotion of independence andpartnership-working is integral to this post.

Key WorkingRelationships
The post holder will be required to maintainconstructive relationships with a broad range of internal and externalstakeholders.
Participate in relevant internal and externaltraining which will be set out by the Personalised Care Institute.
Work closely with the Operations Manager;ensuring the appropriate level of priority is assigned to each task.
FunctionalResponsibilities
Identify and manage a caseload of care packagesto support our most vulnerable patients.
Process all requests received from relevantprimary and secondary care professionals.
Co-ordinate the MDT to provide the best accessand service for the patient.
Provide timely reports on patient progress andmaintain excellent communication with all relevant stakeholders.
Attend all necessary meetings to support patientcare requirements.
InformationManagement
Contribute to effective information managementwithin the team including collation, analysis and reporting of data to supportDES targets in a timely fashion.
Participate in relevant internal and externalgroups to provide relevant information analysis and support.
Planning andOrganisation
Produce and review care plans for identifiedpatients.
Capture detailed information from all involvedin the patients care to ensure informed decision making is possible.
Develop and maintain tracking tools to ensurecare plans are kept on track.
Communication
Establish communication with the OperationsManager, Practice Manager and other managers within the organisation and thewider Healthcare community to ensure that services are integrated.
Ensure that good practice is shared within theorganisation as appropriate.

Excellent written and verbal communication skills Excellent organisation and time management skills with the ability to manage multiple tasks effectively and to changing deadlines. Ability to communicate effectively at a range of levels including MDT meetings and Practice meetings. Ability to analyse and interpret patient data Advanced keyboard skills using a range of software Ability to work on own initiative and organise own workload with minimal supervision Ability to multi-task Self-directed professional with the ability to cope under pressure working to multiple deadlines Highly motivated Enthusiastic and friendly approach Team player Willingness to undertake a DBS (previously CRB) check Willingness to undertake CPD

Experience of processing patient care letters within a Health Care environment Experience of effective working within an MDT with the purpose of achieving a positive outcome for the patient Previous experience of working in a busy confidential office environment Experience of supporting people, families and carers in a related role Experience of working within General Practice Experience working in a General Practice as a Medical Secretary Experience working in a General Practice within the Reception Team
