Job summary

We are looking for an experienced Practice Nurse to join our nursing team.

We are a multidisciplinary Practice that includes GPs, ClinicalPharmacists, Practice Nursing, Health Care Assistants, and Physiotherapists.

Main duties of the job

To be fully proficient in Chronic disease management, vaccinations andimmunisations, cervical screening, contraceptive reviews, wound care includingcompression, as well as other general treatment room duties.

To provide support to junior members of the nursing team includinghealthcare assistants and will direct their work to ensure that the Practicesprovide high quality care.

You will oversee infection control standards and cold chain compliancy.

Oversee recall systems for chronic diseases, cervical screening,immunisations, and other key areas as relevant to Practice Nursing

Delivery effective implementation of management of patients with ChronicDiseases as detailed by the Quality and Outcomes Framework, local qualityschemes and national quality schemes

Support the implementation of evidence-based guidelines in conjunctionwith the wider team

Encourage the development of integrated services within Primary Care toinclude integrated practice-based teams and multidisciplinary working

Oversee infection control and cold chain systems to ensure compliancy

Ensure that all nurse-related Care Quality Commission evidence tableindicators are compliant, demonstrating evidence, review, and audit cycle

Job responsibilities

Participate in an annual appraisal

Actively participate in clinical and practice meetings

Develops clinical protocols for the surgery and the team.

Clinical duties

Provide Chronic disease management for Diabetes, COPD, Asthma, andCHD/CVD

Perform cervical screening and promote breast awareness

Administer adult and paediatric injections

Provide adult and child immunisations ensuring full achievement oftargets

Ensure safe storage, rotation, and disposal of vaccines and drugs

Undertake venepuncture and electrocardiography

Undertake wound care/ management / dressing techniques

Undertake leg ulcer management, dopplers and compression bandaging

Provide advice on lifestyle issues, assisting and supporting patients inchanging their unhealthy behaviours and refer appropriately

Participate in the smoking cessation service

Deliver travel health care, including vaccinations and advice forpatients

Recognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies, and be aware oflocal, referral procedures

Work with safeguarding teams where relevant

Ensure that current evidence-based approaches to patient care arestandard practice

Apply infection control measures when collecting and handling oflaboratory specimens, disposing of waste materials, dealing with blood and bodyfluids spillages and report and treat sharps injuries

Undertake first aid and management of emergencies, burns,haemorrhages, hypo/hyperglycaemic emergencies/asthma attacks

Provides a comprehensive family planning service

Audit outcomes of care against standards and initiate change as necessaryensuring follow-up audit cycle

Maintain and update disease registers

Qualifications

Essential

Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD Wound dressing and leg compressions Cervical screening qualification Venepuncture and electrocardiography

Desirable

Previous experience of Care Quality Commission evidence table to achieve nursing-related indicators to a high standards Stable career history showing clear commitment and progression Independent prescribing qualification
