Job summary

The role of CommunityHealth and Wellbeing Workers (CHWW) is a relatively new and innovative way ofreaching people in the community who can sometimes be underserved in health andcare services. Thisrole is non-clinical. The aim is to reach local people in theirneighbourhood ( at their front door) to promote equity of access to healthand care services. The CHWW performs a variety of duties to improve the healthand well-being of the communities they serve, and they support access toservices. They play a significant role in identifying unmet needs and referringor signposting to appropriate services.

As a CHWW you will be basedwithin Aldershot Primary Care (PCN) working on behalf of the four practices ofAldershot, working alongside the PCN teams and local partners. The outreachwork of a CHWW is essential in identifying unmet needs in three local wards inAldershot and Farnborough where there are significantly higher levels ofdeprivation and inequalities in health experienced.

About us

Salusis a GP Federation which was formed by the local GP practices within North EastHampshire and Farnham. This Includespractices located in Aldershot, Farnborough, Fleet, Farnham & Yateley. We hold several contracts supporting primarycare to deliver services on behalf of those practices, helping them work bettertogether and with other healthcare providers leading to improved patientservices and developing new ones due to constant changes in the Primary Careenvironment.

Wealso assist practices with admin, operational and HR support and facilitateshared learning across localities. Byengaging with the public, we also help ensure that services are designed anddeveloped with patient and public involvement.

The staff at Salus Medical Services have awealth of experience working in general practice and therefore a goodunderstanding of the opportunities and challenges faced.

Job description

Job responsibilities

The following are the core responsibilities of the CHWW. There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels.

Main Duties and Responsibilities:

Meet people on a one-to-one basis mainly via household visits in specific areas (usually visiting homes in a few streets close together). Maintaining and managing a caseload. Once in post, each full-time CHWW will aim to make at least 100 household visits every month. CHWWs will take a personalised approach by considering individual needs, circumstances and personal stories. This role will support peoples holistic needs by linking people with support available in their community, as well as in the health service. Safely manage self in the community and during outreach by following the PCNs lone working policy.

Health promotion and health education

Deliver personalised health promotion and, where necessary, health literacy support to families. Support provision of lifestyle and health improvement advice and information for example on stopping smoking, support to manage alcohol consumption, healthy food and weight, access to talking therapies and promote physical exercise. Identify carers and signpost them to get registered via their GP Practice. Provide advice to households, to support the uptake of health checks, screening and vaccinations. Training will be provided to ensure documentation is maintained to support measuring data outcomes . This includes using technology to manage information and communicate information. Partnership working Work as part of and support an existing multi-disciplinary team. Maintain links with the multidisciplinary team including Social Prescribers, Local Councillors, GP practices, Voluntary Sector, Housing, Social Care, Community Leaders, Community Pharmacists, and Care Coordinators for Rushmoor. To attend occasional partnership meetings and advocate for where there may be shared needs and concerns of residents.

Support

Develop meaningful therapeutic relationships with the local community. Be a friendly source of information about well-being and prevention. Offer informal counsel, actively listen and be empathetic. Support residents to access services available locally to promote health and well-being. Build trust with the person, providing non-judgmental support, respecting diversity and lifestyle choices. Work from a strength-based approach focusing on a persons assets rather than deficits (focus on what is positive and available to them rather than problems). Respecting the privacy, dignity, needs, and beliefs of patients, carers, and colleagues by behaving in a manner that is welcoming, non-judgmental, and respects their circumstances, feelings priorities, and rights.

Community Engagement

Engage with the community to gain feedback about new and local health services Facilitate networks within communities to strengthen sources of informal support. Identify and advocate for the needs of individuals and the community by liaising between the health service and the community. Join and support local community outreach work.

Continuing Professional Development

Each CHWW will be allocated a line Manager to support them with continual personal and professional development and they will also be able to access clinical supervision to ensure patient needs are met, as far as is possible, of those they identify who want support. Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training, and health and safety.

Mandatory training will be provided before working with people.

Feedback from service users.

Number of referrals to community services as required.

A/E attendance and non-elective inpatient admissions from areas with CHWW

999 calls from areas with CHWW
