Job summary

Location: Aylesbury

We have a fantastic opportunity to create a dedicated review team for people with a learning disability with complex needs. The team will have social workers, health specialists and commissioners who will work closely to provide a programme of targeted reviews for high cost clients. The reviews will aim to maximise the potential for independence for the individual and ensure value for money for the public purse.

These Advanced Practitioner posts are fixed term contracts/secondments for 2 years with a workbase in Aylesbury.

Main duties of the job

The team will work closely with each other and the All Age Disability Team in Integrated Commissioning, and colleagues in ASC Operations and finance teams.

Specific activities in the roles will include:

Ensuring providers are clear about the Councils expectations for them to achieve specified outcomes from core support and additional hours Providing a robust challenge process with providers in regard to cost and the use of additional hours Working with social workers on an holistic approach to the range of services individual clients are receiving and the cost effectiveness of this Effective utilisation of the CareCubed costing tool to support dialogue around cost and value for money (training will be available). Working with commissioners to identify the gaps in provision to meet the needs of disabled adults with complex needs in-county.

Any-Desk:

As an any-desk worker, youll need to be connected to our network to access digital folders and resources, but this can either be from home or an office location when required. You will be required to work from the office a couple of times a week, depending on your role and team requirements.

Job responsibilities

You will need to demonstrate the following confidently and effectively:

A strong understanding of residential and supported living learning disability services. Previous experience of working in this or a similar sector would be an advantage. Experience in analysing and adjusting support packages for people with complex needs to ensure that providers enable skills development and support complexity appropriately. An understanding of what good quality care looks like for individuals with complex learning disability and or Autism. Experience of designing, leading, and managing change, both for individuals or groups of people. Able to influence others, build and manage effective relationships across health and social care . Comfortable and confident engaging in complex or challenging negotiations. Able to use professional curiosity and creativity to develop new and innovative solutions. Strong verbal, presentation and written skills with confidence in delivering information concisely
