Job summary

The post holder will be part of a team dedicated to ensuring patient experience and engagement informs the care provided by the Trust.

The post holder will act within their professional boundaries to support people with long term health conditions or cancer to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role will assist to increase patient ability to access and utilise community support whilst providing access to self-management, education and peer support.

It is key within this role to work closely with the Trust's multidisciplinary teams, as well as our patients, relatives and carers and with the wider community, to ensure we are hearing what our community have to say and work together to achieve the best possible patient experience.

A key part of the role will be to promote awareness days, by networking with our Clinical Nurse Specialists across the Trust, to facilitate events and lead on support groups for the trust and the local community.

Main duties of the job

Establish network relationships with existing Cancer Support Workers, Health Coaches, and other relevant Health Professionals, within Cancer Services and the wider healthcare community including partners in primary care, community services and mental health services, to ensure wrap around care of the individual.

Provide regular support to patients, relatives and carers with long term health condition(s) including cancer, assessing their holistic needs, and supporting them to self-manage by providing the relevant information and advice and signposting them to other resources and services available to meet their identified needs. Visit patients who have been in hospital for over 4 weeks, to assess their health and well being and provide support/advice as required.

Following all clinical input, coordinate care with the person living with long term condition(s) so they can access the right help at the right time, providing a seamless experience of care. Check that the person is able to self-manage and provide a flexible and personalised response.

Support those being treated for or living with long term condition(s) including cancer, to navigate the complex health and social care system, by building relationships with professionals and facilitating access to these services, including generating referrals on the person's behalf where necessary.

Job responsibilities

The post holder will be organised and able to prioritises their workload and be flexible in response to completing demands. Work collaboratively with colleagues to deliver and evaluate diversity network events and activities which support the delivery of people living with long term health condition and cancer.

You will Plan activities across multiple workstreams, adjusting plans and priorities as required to meet local, regional, national requirement.

Must be able to apply own judgement and initiative in areas of poor data quality or complex guidance, demonstrating awareness in line with organisation priorities and objectives trends, themes.

Considers a variety of issues and proposes solutions when dealing with complex enquires by taking appropriate action as necessary or liaising with others/redirecting queries as appropriate.

Undertakes any project work as required, including obtaining information from the internet, analysing, collating, and reporting of data to produce reports, spreadsheets and presentations utilising relevant IT systems, when required. Develops and maintains spreadsheets and databases, information systems and internal forms. Uses a variety of IT packages including Microsoft Word, Excel, Access and PowerPoint in order to complete tasks.

Create appropriate content for a range of communications channels, including the patient engagement, intranet site and social media. Work with members of the engagement team to develop a patient engagement section of the trust intranet.
