The Urology Remote Monitoring Nurse will provide support and guidance to patients who will be discharged back into the boroughs of North East London (NEL); Tower Hamlets, Waltham Forest and Newham. The post holder will also hold a managerial role of overseeing the uro-oncology team across Barts Health, including leading/coordinating projects and initiatives across the network. The post holder will have excellent communication and influencing skills to build relationships with senior managers and clinicians across NEL.

You will be expected to play an active part in supporting the quality of care and the experience of prostate cancer patients through clinical practice, nurse led clinics, education, research and audit.

Main duties of the job

The Urology Remote Monitoring Nurse will provide support and guidance to patients who will be discharged back into the boroughs of North East London (NEL); Tower Hamlets, Waltham Forest and Newham. The urology remote monitoring nurse will facilitate individuals with prostate cancer in the community have the correct information, care and rehabilitation required to support them following transfer onto a supported self-management or shared care pathway.

Based at Barts Health NHS Trust, the post holder will engage and collaborate with local cancer and generic health and social care services, service users, the voluntary sector, and outreach/support groups in order to identify and improve patient experience for individuals living with cancer in North East London. Partnerships will be developed with GP services within North East London in order to maximise reach and accessibility of the cancer case manager service. The post holder will have excellent communication and influencing skills to build relationships with senior managers and clinicians across NEL.

The Urology Remote Monitoring Nurse will also hold a managerial role of overseeing the uro-oncology team across Barts Health, this will involve line management, regular performance reviews and dissemination of organisational information including leading/coordinating projects and initiatives across the network
