Job summary

Are you an enthusiastic and passionate Assistant Practitioner looking for a fresh start in 2024? Come and join Sirona in a role where you can make a real difference to the community you live in.

You will work in a team with other multi-disciplinary professionals including Nurses, Physiotherapists, Occupational Therapists, Administrators and other support staff, on a rota system between the hours of 8am-8pm over a 7 day working period.

You will need excellent communication and clinical skills and have experience of working within a team. You will be required to use electronic patient record systems, in addition to this you will have good time management and be able to prioritise your own work load.

A full driving licence and access to a vehicle is required for this role and whilst we are looking for full time hours, we can consider part time hours.

We have Assistant Practitioner vacancies across South Bristol and Weston & Worle.

Main duties of the job

Clinical monitoring of acutely unwell patients to support prevention of admission to hospital when clinically safe to remain at home.

Support the early discharge of patients from hospital: including providing holistic initial assessments of functional, mobility and medication needs.

Carry out the relevant clinical competencies required of the post , wound management, administration of medications including insulin to non-complex patients (under direction of registered nurse), catheterisation (supra-pubic/male/female), equipment and mobility assessment, and functional assessments as outlined in Sirona Assistant Practitioner competency framework.

To undertake and complete appropriate training and competencies in order to carry out the role of Assistant Practitioner.

Implement the Sirona strategy and organisational values.

Qualifications

Essential

Successful completion of relevant Foundation Degree modules / Diploma in Clinical Health and Social Care / diploma level 3 or equivalent.

Skills, knowledge and experience

Essential

Experience of working in a health or social care setting providing direct patient care Ability to carry out the relevant clinical competencies required of the post: clinical, nursing and therapy based. Ability to assess patients needs using EMIS templates, assessment tools and produce treatment plans. Ability to manipulate fine tools, for Venepuncture and other clinical tasks Effective communication skills both written and verbal. Demonstrate a sound understanding of Clinical Governance, Clinical Risk and to use Evidence Based Practice in order to provide optimum care to patients.
