Riverside HealthPartnership serves approximately 23,000 patients in Bassetlaw. We are a busy 4site practice with a team of 12 GPs, 5 ANPs, 8 Practice Nurses and 3 HealthCare Assistants.

We are a consistentlyhigh performing GP training practice operating on SystmOne clinical system. Thesuccessful applicant must have their own transport as working across fourssites may be necessary.

Main duties of the job

It would be essential for the successful applicant to have experience and hold the necessary qualifications in:

Chronic Disease Management

Travel and Childhood Immunisations

Cervical Cytology

General treatment room

Job description

Job responsibilities

To provide nursing services to thepractice population by evidence based, quality focused care. To assess, plan, implement and evaluatecare. To ensure clear channels ofcommunication exist between clinical, administrative and community promote a positive and friendly image toour patients and other professionals.

All employees are expected to displaya high standard of teamwork. Thisincludes cooperating in undertaking the work of absent colleagues or assistingothers where the workload is particularly high.Accordingly, there may be a requirement, from time to time, to undertakeother duties.

Principal Responsibilities

To provideassessment, treatment, screening and health education services and advice,working in accordance with set protocols, guidelines and NMC Code of Conduct.

Providenursing service/treatments to patients independently or in participation withgeneral practitioners with reference to aforesaid agreed protocols andguidelines.

Specific Duties

Nursing procedures to include, but not limited to - Wound management Hypertension reviews Routine injections as prescribed Cervical Smears Childhood and Adult immunisations,including reading and reviewing Patient Group Directives (PGDs) and PatientSpecific Directives (PSDs) Well Person Health Checks includingForeign Travel Counselling, NHS Health Checks, Lifestyle and New Patient Checks ECGs Ear syringing and assessment of ears Spirometry Obesity and weight/diet management Family Planning includingassisting in Implants & Coil fittings & Sexual health Chronic disease management andprevention ~ Diabetes, Hypertension, Hypothyroidism, Stroke, Epilepsy, IHD,COPD and Asthma( would be advantageous) Leg ulcer management Minor Injuries and Minor Illness INR Monitoring and Dosing using(INRStar N3) CVD risk assessments and Q riskinterpretation and assessments. Referrals Podiatry, Counselling,Audiology, etc Ensuring all specimens are checked and sent tolaboratory as required

You will be fully conversant in QOF, read coding and upholdstrategies to provide a holistic combined healthcare for the patient anddemonstrate a willingness to participate in research/audit within the practiceteam, and the use of SytmOne.

You will be aware of DES/LES policies and apply and produce,alongside the Practice Manager, protocols that reflect the guidelines that areworkable in the practice.

Participate in the education of students of alldisciplines.

To be responsible for the efficient running of the treatmentroom and equipment and to ensure the clinical areas are at all times safe forstaff and patients.

To adhere to strict Infection Control in line withguidelines and policies.

Advise patients and relatives/carers on general health careand ailments with referral to GPs as necessary.

Administrative responsibilities

To maintain accurate, comprehensiverecords of all consultations and treatments in the patients notes, bothwritten and computerised, awareness of QOF, read codes and alerts needed forstandardised care. All associated nursingadministration eg. Vaccine forms, Travel Health Questionnaires, protocolreview and assisting Reception with queries. To access and file cytology results. Manage clinics effectively. To take personal responsibility forown personal/professional development, providing sufficient evidence of theachievement of set objectives through individual performance and the reviewprocess. Ensure accurate completion of allnecessary documentation associated with patient health care and registrationwith the practice. Help with collection of statisticalinformation required for QOF & QIPP. Attend and participate in practiceand staff meetings as appropriate. To take prompt action to defusesituations of complaints and be equipped with the knowledge to advise patientsand relatives/carers about the complaints procedure. Maintain appearance and behaviour inkeeping with a member of the nursing profession and in accordance with codes ofconduct. Support and co-ordinate healthpromotion campaigns, flu, no smoking, healthy lifestyles, etc Maintain effective liaison withother agencies and staff concerned with patient care and with other disciplineswithin the practice, with appropriate regard to confidentiality.

Treatment Room supplies andequipment

To have working knowledge ofequipment used in the surgery. Ensuringthat any faulty equipment is taken out of service for repair and reported tothe nursing team for information and Practice Administrator for action. To be aware of Infection Control andupdated policies. In participation with the Nurse Leadto ensure the maintenance of equipment and stock levels in relation to patientcare. To regularly update knowledge andinformation through use of medical journals, training, courses, etc. To maintain the cold chain in fridge/fridges and the daily recordings. Any other duties as reasonably requested by theDoctors, Practice Manager and Nurse lead, providing these duties are within thescope of the role and that the individual is trained and capable of such duty.

The role of the Practice Nurse is an evolving one and it isanticipated that the post holder will be responsible and reasonably flexible inthe interpretation of the above job description.
