Job summary

To provide an effective Community Nursing Service for children and young people with moderate/severe Learning Disabilities, their families/carers within the Bassetlaw locality.

Main duties of the job

The post holder will work as a member of the Children's Community Learning Disability Nursing Team with specific responsibility for assessing, planning and supporting with the delivery of individual care plans for children and young people with moderate/severe Learning Disabilities in their homes, school and community settings To function as case holder for children and young people with moderate/severe learning disabilities, supporting and supervising health care support workers in the team. Provide community visits and liaise with relevant agencies to ensure effective review and delivery of care. Co-ordinate and develop care plans for individuals utilising clinical skills and knowledge of appropriate assessment tools, nursing process and therapeutic interventions in line with best practice and in accordance with Trust Procedures and guidelines.

Job description

Job responsibilities

The post holder will be expected to demonstrate high levels of autonomy and decision- making skills and undertake comprehensive assessments related to complex behaviour/toileting/sleep and health concerns. Develop and deliver therapeutic interventions within a multi-agency framework for the benefit of children, young people with moderate/severe learning disabilities and their families/carers The post holder will work in partnership with and contribute to the development of a multi-agency team approach to promote and optimise all aspects of emotional health and wellbeing for the learning disabled child/young person. The post holder will offer support and specialist learning disability nursing advice and knowledge to other registered nurses and healthcare workers within the trust, colleagues in education, social care settings and voluntary sectors in relation to clinical care and learning disability related topics. First level RNLD/RNMH ENB 998 or DC1/2

Experience

Proven minimum 3 years post qualification experience. Experience of multi-agency work. Experience of working with children/young people and their families around a range of health and behavioural needs. Experience of managing vulnerable children's needs Experience/knowledge of safeguarding issues in relation to children and young adults
