As a Health and Wellbeing Coach you will work as a key part of the primary care network (PCN) multi-disciplinary team. You will consider the whole person in addressing existing issues and encouraging the proactive prevention of new illnesses, promoting lifestyle medicine approaches.

Health coaching revolves around using your coaching skills to support people with lower levels of patient activation to develop the knowledge, skills, and confidence to manage their health and wellbeing, whilst increasing their ability to access and utilise community support offers.

Main duties of the job

The post holders will work in partnership with their clinical and non-clinical colleagues, management support and the wider PCN to ensure the role delivers the best possible outcomes for our patients.

The role is patient facing, varied, rewarding and sometimes challenging and may include supporting self-management education, peer support, case management and facilitating group consultations. You will take an approach that is non-judgemental, based on strong communication and negotiation skills. You will support personal choice and positive risk taking, while ensuring that patients understand the accountability of their own actions and decisions.

As a whole your role and skills will support and encourage the prevention of developing further illness, or the deterioration of existing long term conditions.

Job responsibilities

Although the successful candidate may be required to work in any Lowestoft Practice. This position will mainly be based in Alexandra Road and Crestview Surgery.

Coach and motivate through multiply sessions using personalised goal setting to implement a patients personalised health and care plan for long term lifestyle and behaviour change.

Identifying frequent attenders and work with patients with lower activation scores to understand and increase their level of knowledge, skills and confidence to self-management their health conditions.

Pain management psychological element

Mental health (well managed)

Working with carers

Referring to Social Prescribing Link Workers and the broader PCN MDT to enable patients to take control of their health and wellbeing.

Supporting return to work.

Supporting participation in community activities/exercise to improve mental and physical wellbeing.

Addressing loneliness

Supporting recovery from drug and alcohol addiction.

Lifestyle group education sessions / webinars

Virtual / group consultations

To be responsible for day-to-day planning and personal caseloads

To develop yourself and the roll through training as well as coaching and clinical supervision.

Identify when patient needs are beyond the scope of a HWbC and refer to the appropriate health professional or organisation.

Proactively work with referral partners to maximise appropriate referrals and to promote the service.

Person Specification

Experience

Essential

Experience of working within a social care and/or health setting, working with multi-disciplinary teams . Demonstrable experience of effective planning and organisational skills to deliver targets to deadlines

Desirable

Experience of multi-agency working and signposting to appropriate support Experience of working with people with diverse health and social care needs Experience of using recognised assessment tools Experience of mental health either as service user, carer, worker or volunteer Experience of using Patient clinical systems

Qualifications

Essential

Educated to GCSE or equivalent Be willing to attend a 4 day training course delivered by an accredited Personalised Care Institute training provider.

Desirable

NVQ 3 or equivalent and/or relevant basic/first level professional qualification

Knowledge and Skills

Essential

Excellent verbal communication skills with the ability to communicate effectively at all levels Good technical literacy of Microsoft Applications Word, Excel, and Access etc. Good interpersonal skills including listening and displaying empathy Creative, flexible and imaginative approach to working with people with diverse support needs Understanding of the barriers people face to accessing services and how to overcome them Ability to support and motivate people to make sustained changes in their lives Ability to reflect on and share practice with peers Have access to a car and being in possession of a full current driving license and business insurance

Knowledge of a range of community groups and services which support wellbeing Knowledge of a range of interventions which support behavioural change Motivational Interviewing Awareness of relevant Health and Social Care legislation and a developed knowledge of crisis intervention Knowledge of the safeguarding interventions and awareness of the Mental Capacity Act Awareness of data protection and confidentiality issues
