Job summary

This is an exciting opportunity to work within primary care whilstdelivering innovative care to the wider community. The role offers anopportunity for a physician associate to develop as a primary careclinician.

The physician associate will work within a GP surgery to deliveryacute and chronic disease management. As part of the wider multidisciplinaryteam, we envisage the physician associate will be involved in acute visits,frailty management, chronic disease management, care home visits, advanced careplanning and palliation, whilst working within their scope of practice.

Appropriate supervision and support will be available to PhysicianAssociates.

Opportunities to take part in quality improvement projects will beavailable.

We welcome applications from experienced as well as newlyqualified physician associates for this role and will continue to support PAswith access to monthly teaching session, CBD and development. Peer to peersupport and mentorship will be available for all PAs employed within the PCN.For those new to primary care the engagement with preceptorship would besupported.

Main duties of the job

Provide first point of contact for patients presenting withundifferentiated, undiagnosed problems, utilising history-taking, physicalexaminations and clinical decision-making skills to establish a workingdiagnosis and management plan in partnership with the patient.

Examine, assess and diagnose patients and provide clinical care/management as required.

Review, analyse and action diagnostic test results.Collect/request pathology specimens as required.

Support patients in the use of their prescribed medicines orover the counter medicines (within own scope of practice).

Deliver integrated patient centred-care through appropriateworking with the wider multi-disciplinary team and social care networks.

Ensure continuity of care, arranging follow-up consultations orreviews as necessary.

Provide routine care to patients as required in accordance withclinical based evidence, NICE and the NSF.

Utilise clinical guidelines, promote evidence-based practice andpartake in clinical audits, significant event reviews and other research andanalysis tasks.

Ensure that they adhere to the relevant patient group directivesand local clinical pathways at all times.

About us

The physician associate will have access to both an education andclinical supervisor within their GP base. The level and type of supervisionwill be dependent on the post holders skills and knowledge and determined bythe organisations clinical governance arrangements.

Main Purpose of the Job

The physician associate will be required to work autonomously inthe practice, providing a range of services such as assessment, diagnosis,treatment, telephone triage, and clinical decision-making and referringpatients appropriately.

General practice

Review the effectiveness of the treatment provided, makingchanges where necessary to improve patient outcomes

Deliver integrated patient centred-care through appropriateworking with the wider multi-disciplinary team and social care networks.

Play an active role in the management of patients with long termconditions

Deliver opportunistic health promotion where appropriate

Ensure continuity of care, arranging follow-up consultations orreviews as necessary.

Liaise with external services/agencies to ensure that the patientis supported appropriately (vulnerable patients etc).

Understand practice and local policies for substance abuse andaddictive behaviour, referring patients appropriately.

Provide routine care to patients as required in accordance withclinical based evidence, NICE and the NSF.

Utilise clinical guidelines, promote evidence-based practice andpartake in clinical audits, significant event reviews and other research andanalysis tasks.

Ensure that they adhere to the relevant patient group directivesand local clinical pathways at all times.

Support the clinical team with all safeguarding matters, inaccordance with local and national policies.

Carry out immunisations as required

Chaperone patients where necessary

Maintain accurate clinical records in conjunction with currentlegislation.

Ensure read codes are used effectively.

Work within scope of practice, and seekassistance/support when required
