Job summary

This is an exciting opportunity to join our growing team based at Bransholme Health Centre, Hull. You will be askilled Registered Nurse, Registered Paramedic or Registered Physiotherapistwith the appropriate attitude, skills and knowledge base.

In addition, you may hold a currentqualification at degree level in management of minor illness, a Bachelorsdegree or working towards a Bachelors degree, or currently be undertaking aMasters level programme in Advanced Clinical Practice. You will deliver quality holistic care andtreatment to the Practice population.

Main duties of the job

Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnoses within your sphere of competence

Clinically examine and assess patient needs froma physiological and psychological perspective and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluateinterventions/treatments for patients with complex needs

Utilise your clinical supervisor/mentor andother clinical staff appropriately to ensure safe and effective patient care,demonstrating awareness of your own limitations

Provide safe, evidence-based, cost-effective,individualised patient care within the surgery, patients own home or otherenvironment where patient care is carried out

About us

James AlexanderFamily Practice is owned and run by 2 GP Partners who both follow portfoliocareers and appreciate the importance of personal interests and family life.

We have 4 salaried GPs and 11 Clinical Practitioners, 9 of whom are prescribers.

We have a strongnursing team with 5 Practice Nurses and 5 Health CareAssistants.

We have recently undergone extensive organisational changes to help us future proof ourorganisation, create efficiency savings and streamline services. This helped strengthen our non-clinical team,with our current non-clinical team working effectively, providing high qualitycare and support to all our clinical staff and patients.

We have alsocomplimented our strong clinical team with 2 dedicated Clinical Pharmacists andPharmacy Technician.

Job responsibilities

Clinical Responsibilities:

Assess, diagnose, plan, implement and evaluatetreatment/interventions and care for patients presenting with anundifferentiated diagnoses within your sphere of competence

Clinically examine and assess patient needs froma physiological and psychological perspective and plan clinical care accordingly

Assess, diagnose, plan, implement and evaluateinterventions/treatments for patients with complex needs

Utilise your clinical supervisor/mentor andother clinical staff appropriately to ensure safe and effective patient care,demonstrating awareness of your own limitations

Provide safe, evidence-based, cost-effective,individualised patient care within the surgery, patients own home or otherenvironment where patient care is carried out

Refer patients directly to other services/agenciesas appropriate, utilising practice and local guidelines and in a timely manner

Proactively identify, diagnose, monitor andmanage treatment plans for patients at risk of developing a long term conditionas appropriate

Diagnose and manage acute conditions,integrating both drug and non-drug based treatment methods into a managementplan

Prescribe and review medication for therapeuticeffectiveness, appropriate to patient needs and in accordance withevidence-based practice, national and practice protocols and within own scopeof practice. Utilise Patient GroupDirections for the administration of medication as appropriate

Work with patients in order to supportcompliance with and adherence to prescribed treatments

Provide information and advice on prescribed andover-the-counter medication on medication regimens, side-effects andinteractions

Prioritise health problems and interveneappropriately to assist the patient in complex urgent or emergency situations,including initiation of effective emergency care

Support patients to adopt health promotionstrategies that promote health living and to apply principles of self-care

Recognise, assess and refer patients with mentalhealth needs as appropriate

Leadership responsibilities:

Other responsibilities:

Recognise and work within own competence and inaccordance with the professional code of conduct of the Nursing and MidwiferyCouncil (NMC) or Health and Care Professions Council (HCPC) as appropriate

Monitor the safety and effectiveness of ownclinical practice through quality assurance strategies such as the use of audit,mentor feedback, case review and peer review

Maintain accurate and complete documentation andrecords utilising IT as appropriate and in accordance with practice protocol

Use own judgement, resourcefulness and commonsense

Deliver care according to evidence basedpractice, following agreed protocols, local and national guidelines

Understand the applicable legal policy thatsupports the identification of vulnerable and abused children and adults, beingaware of statutory child/vulnerable adult health procedure and local guidance

Work within policies regarding family violence,vulnerable adults, substance abuse and addictive behaviour and refer asappropriate

Contribute to achievement of the highestpossible quality standards such as the QoF and other Key Performance Indicatortargets

Attend in-house governance, educational andstaff meetings as appropriate

Assist with the training of other staff membersand also with the education of doctors, nurses and other health professionalsin training as appropriate

Pro-active engagement with the practicepopulation and wider community to promote healthy living and encourage uptakeof services. This may include visits tolocal schools, community centres and other groups as necessary

Undertake additional tasks as required withinyour general role as a Clinical Practitioner and senior member of the team

Learning and development:

The post-holderwill participate in any training programme implemented by James AlexanderFamily Practice as part of this employment, such training to include:-

Participation in individual performance reviews,including maintaining a record of own personal and/or professional development

Working in conjunction with senior clinicians,assess own learning needs and undertake learning as appropriate

Participate in continuing professionaldevelopment opportunities to ensure that up to date evidence based knowledgeand competence in all aspects of the role is maintained

Person Specification

Qualifications

Essential

You should be a skilled Registered Nurse, Registered Paramedic or Registered Physiotherapist with the appropriate attitude, skills and knowledge base.

Desirable

Although a dependent practitioner, you should be able to work independently and make independent decisions enabled by a collaborative and supportive working relationship with your clinical supervisor. You will need to demonstrate safe and effective clinical decision-making and expert care, including assessment, diagnostic and management skills. As well as dealing with acute illness, experience of being involved in the management of long term conditions would be advantageous.
