Job summary

An opportunity has arisen for a Heath and wellbeing support worker to join the Tobacco Dependency Team within Sherwood Forest Hospitals NHS Foundation Trust. We are looking for an enthusiastic and highly motivated individual to join our established, friendly and dynamic team.

Experience of previous work as a Tobacco Dependency Advisor is desirable but not essential as full induction and training will be provided. We are looking for someone who is patient care focussed, non-judgemental, with initiative, time management skills and the ability to work safely within their scope of practice. Communication and interpersonal skills are also essential to this role, in order to support patients to make positive lifestyle changes.

The role requires the successful applicant to be flexible and adaptable with good organisation skills. You will work closely with staff and patients in a multi-disciplinary team setting, supporting the provision brief advice, Nicotine Replacement Therapy (NRT) and onward referrals to community provider's post-discharge.

Main duties of the job

Support the delivery of actions and activities to ensure the effective implementation of NICE PH48 and the MECC agenda Provide Smokefree and MECC brief interventions at the bedside to support individual patients as required Support patients and staff to make informed choices about their health and wellbeing and supporting them in initiating and sustaining appropriate behavioural changes to improve their health. Work with the Health and Wellbeing Lead and the Smokefree Team to facilitate the referrals made. Act as a role model to front-line staff in the engagement and delivery of brief interventions to patients Support front-line staff to develop the knowledge and confidence to effectively offer Nicotine Replacement Therapy and e-cigarettes to patients Develop strong working relationships with team members, ward staff and the local stop smoking service Support the delivery of training courses or updates, for example brief interventions to ward staff and to support the delivery of health and wellbeing study days Participate in the organisation and delivery of health promoting events Travel is required across our three hospital sites To embody the Trust's CARE values in daily duties
