Job summary

The aim is to provide an extended service to hostels and street homeless residents within the London Borough of Brent. The role will include a range of activities addressing the client's health and wellbeing, the development of pathways into specialist services, training of hostel and partnership outreach staff to manage residents out of hours, and improving the overall health inequalities via;

Health promotion General Health Screening Engagement in Primary Care services and facilitating access to organised healthcare

The purpose of this role will be imperative in improving integrated pathways to prevent fragmented services that are efficient and effective. The role is specific to supporting those who want to improve health outcomes, especially for those who continually consume alcohol where traditional structured treatment will not be appropriate. The role will require peripatetic nurse-led general health clinics within hostels, supported through general practice.

The aim of the service is to help those with a range of health needs, including drug and alcohol use, access appropriate treatment and address clients' physical and mental health.

Furthermore, the service aims to reduce alcohol-related hospital admissions and health inequalities and improve pathways with community and hospital services.

To ensure that the care and management of this client group are delivered in compliance with HCC Quality Standards.

Main duties of the job

Main Responsibilities:

To provide specialist drug and alcohol outreach clinics for local hostels, GP surgeries and organized health services in Brent, including follow-up referrals of drug and alcohol clients who are hard to engage. Work closely with GP Complex. Work closely with GP Complex Patients Management team, taking on/ follow up referrals. To work as a member of the multidisciplinary team in the management and treatment provision of a range of specialist services to problem drinkers and problematic drug takers residing in hostels, out of reach areas. To assess the client's general health and provide early intervention treatment, sign posting, and referral to other organized health services. To develop and maintain pathways into specialist services to ensure the health and well-being of residents are met, including primary care services. To sign post service users to the appropriate health care settings to meet their needs. To be the single point of contact for Primary care services, local Hospitals and other organized health care services.

Identify and refer dependent drinkers and those assessed to need further input into specialist alcohol services.

Person Specification

Education and Qualifications

Essential

oRMN / RN via the NMC professional body oEvidence of continuing personal development and post-registration training in the nursing care, assessment and treatment substance misuse clients.

Desirable

oDiploma in Drugs and Alcohol Studies or equivalent qualification oTraining in Cognitive Behavioural Therapy oCompleted the Motivational Interviewing Training Module

Previous Experience

Essential

oPost-registration experience in Mental or general Health oExperience in senior role and managing staff oExperienced in the supply and administration of Hepatitis vaccines oExperienced and competent in the supply and administration of CNWL Trust Hepatitis Patient Group Directions oExtensive clinical experience oCase management experience oExperience in undertaking Risk Assessments and Risk Management oTeaching and Assessing qualification ENB 998 / SLICE or equivalent eg: mentorship

Desirable

oCommunity Nursing Experience oExperience of working in and delivering services to injecting drug users. oExperience of teaching others and willingness to undertake Teaching and Assessing qualification within six months of commencing post.
