Main duties of the job

Accountable to the Head of Integrated Localities.

Accountable for own practice in line with Trust policies and guidelines.

Nursing Associates are also accountable under NMC code of conduct.

To take responsibility for accepting and implementing patient care as delegated by the Registered Nurse, that is within the Oxford Health District Nursing Service Scope of Practice for Band 4 Assistant Practitioners/Nursing Associates, and where you are signed off as competent in that task.

In collaboration with the Registered Nurse, deliver individual programme of care inpartnership with patients, carers and other health and social care professionals in a response to planned needs.

 It requires strong all-round capability, insights and adaptability in physical and mental health, as well as organisational, communication and inter-personal skills.

- Assessment and planning of delegated care

- Monitoring of long-term conditions and well-being of service users including escalation of interventions when necessary

- Assist with the implementation and monitoring of clinical care and monitor outcomes

- Undertake onward referrals to appropriate services and community-based care support
