Job summary

An exciting opportunity has arisen within The New Surgery Pencoed for a part time accomplished, highly motivated Practice Nurse.

We are a high achieving teaching practice situated within a purpose-built Primary Care Centre with excellent travel links to both Cardiff and Swansea.

You should be able to provide comprehensive high-quality nursing care to our patients. They would need to be skilled in all aspects of General Practice including Chronic Conditions Management (Diabetes, COPD, Asthma, Heart Failure), cytology, Immunizations including childhood, Dressings and a full range of Enhanced services.

We are looking for the right people to join our collaborative, friendly and forward-thinking clinical team. The practice is part of the Bridgend East GP Collaborative and Pen-Y-Bont Health federation.

The salary is negotiable and will be depending on qualifications and experience. The minimum salary £30,000

Main duties of the job

To assist medical personnel in the care of Practice patients to include a full range of treatment room services, preventative care, screening and patient education, childhood immunisations and a knowledge of the range of chronic disease conditions.

The New Surgery has 8,500 patients.

4 GP Partners, 4 Salaried Doctors, 2 practice nurses, 2 HCSW

Modern purpose-built Primary Care Centre with excellent travel links to both Cardiff and Swansea (2 minutes off junction 35 of the M4).

First EPS (electronic prescrining) pratice in South Wales.

Bridgend Vasectomy Clinic

Training practice for GP Registrars, Foundation Doctors and practice Nurses.

Active involvement of the Bridgend East GP Collaborative and Pen-Y-Bont Health federation.

Support froma cluster pharmacist and primary care-based physiotherapy.

EMIS web system

High QAIF achievement

Low home visit rate.

We are a forward thinking, enthusiastic, and well thought of practice. We are located conveniently on the M4 corridor in purpose built premises, providing a high standard of care for patients. We provide the Vasectomy Service to patients in the Bridgend County Borough area under a Service Level Agreement with the Health Board.

Job responsibilities

Job Responsibilities:

Providing assessment, screening and treatment services and health education advice

Participate in the organisation and coordination of the provision of nursing services for the Practice to include the delegation of duties to other Practice nursing support staff as appropriate.

Provide nursing treatments to patients in participation with general practitioners or independently to agreed protocols.

Provide general and specific health screenings to the Practice patients (within agreed protocols) with referral to general practitioners as necessary.

Advise patients on general health care with referral to GPs as necessary.

To provide chronic disease management reviews to ensure that patients in these groups of patients receive an annual review.

The Practice provides a full range of enhanced services including noncomplex wound care and administration of Gonadorelins.

Supplies and equipment Treatment room and other areas

In participation with the Practice Manager, ensure the maintenance of equipment and stock levels relating to patient care.

Pathological specimens and investigatory procedures

Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment so used. Perform any investigatory procedures requested by the GPs.

Administrative and professional responsibilities

Participate in the administrative and professional responsibilities of the Practice team.

Ensure accurate notes of all consultations and treatments are recorded in the patients notes on the clinical computer system as appropriate.

Ensure Practice Nurse path lab in box is reviewed and results actioned within agreed protocols.

Ensure accurate completion of all necessary documentation associated with patient health care and registration with the Practice.

Ensure collection and maintenance of statistical information required for regular and ad hoc reports.

Attend and participate in Practice meetings as required.

Assist in the formulation of Practice philosophy, strategy and policy and develop appropriate protocols.

To Provide a clinical overview of the Health Care Support Workers.

Research projects

Co-operate and participate as required in any research projects within the Practice.

Student/staff education and training

Participate in the education and training of Registrars/Students of all disciplines and the induction of all members of Practice staff with special responsibility for nursing staff.

Liaison

Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the Practice, with appropriate regard to confidentiality.

Professional development

Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
