Job summary

The Clinical Service Manager will help deliver the Community Diagnostic Centre (CDC) Non-Specific Abdominal pathways (NSS) clinical pathway.

The post holder will support clinically with the Lead Consultant and the Nursing team to develop and embed the NSS pathway on behalf of the Trust and system.

This post is funded by the Surrey and Sussex Cancer Alliance (SSCA) for a two year fixed term period. QVH will be the designated hub forSussex for this pathway.

Main duties of the job

To perform at an advanced level within the four broad based competencies of advanced nursing practice:

Expert clinical practice;

Leadership and collaborative practice;

Improving quality and developing practice;

Developing self and other.

Within this framework the post holder will:

Develop consistent approaches for the management of the NSS Pathway through training and education, policy and guideline development, audit, research and implementation of evidence-based care;

Engage directly with clinical medical and nursing teams in the management of patients, providing specialist advice, skills leadership, collaboration and consultancy. Ensuring that patients gain appropriate and timely care;

Autonomous working through provision of initiation of investigations, interpretation of results and optimising patients' condition in discussion with the multidisciplinary team;

Clinical lead for NSS pathway

Act as a nursing professional lead, providing a strong clinical presence, assuring the provision of a safe, effective and evidence based nursing and medical care to patients;

Work closely with the NSS team, actively contributing to the overall management and strategic direction of the service;

To be the lead for designated projects and areas of work which reflect clinical, professional and corporate objectives, which impact beyond the scope of own service/speciality.

Job responsibilities

1. Key result areas:

Expert Clinical Practice

Follow protocols and take full medical histories of patients referred to the NSS pathway

Employ advanced decision making in order to implement appropriate diagnostic plans;

Order investigations including bloods, CT imaging.

To record assessment findings and management plans in a timely manner within the patients medical notes;

Triage and implement a management plan or escalate as appropriate;

To provide relevant individualised health promotion/ education to this patient group;

Use their professional judgement in managing complex and unpredictable care events and capture the learning from these experiences to improve patient care and service delivery;

Present patient to MDT and participate in discussions and provide expertise on plan of care for the patient and oversee MDT processes and appropriate follow up care.

Recognise own limitations and effectively seek appropriate help from a range of multi-agency and inter-professional resources in their practice;

To effectively manage patient enquiries and keep them informed of progress, treatment and involved in decision making;

To initiate patient discharge where appropriate and to write discharge letters to patients and their GPs.

Leadership and Collaborative Practice

To co-ordinate and participate in daily/regular discussions and meetings with the teams;

To be an integral part of the implementation and sustainability of the NSS Pathway

Contributing to reviews of the service and suggesting recommendations where appropriate in accordance with the continual monitoring of the service and outcomes

Supervise and counsel staff on professional and personal matters, monitoring sickness and absenteeism;

To participate in the disciplinary procedure, if necessary; lead the implementation of the Trusts disciplinary procedure or other HR policies within the department as required where necessary

Assess skills and competence of all staff and manage appropriately if they do not meet expected standards

Manage the monitoring of staff sickness and absence

Ensure mandatory training is kept up to date

Support junior team development, education planning, supervising and assessment as required.

Lead on processes pertaining to the recruitment, selection and retention of relevant staff;

To undertake management duties including rostering, monitoring, daily staffing issues and concerns and appraisals.

Improving Quality and Developing Practice

To identify and implement systems to promote the nurse practitioner contribution to the care group and demonstrate the impact of advanced level nursing to the healthcare team;

To provide consultancy services to their own and wider multidisciplinary teams and to enable practice and service development;

To facilitate the development, implementation and review of nursing and clinical policies and guidelines to support practice;

To initiate, guide and support the senior nursing staff in monitoring, evaluating and improving the patient experience;

Assume responsibility for risk issues within the NSS pathway ensuring corrective action is taken and clinical risk is managed;

To implement national guidance and collaboration with stakeholders and surrounding MDT;

To support the development of integrated care pathway implementation within the NSS pathway

To participate in wider meetings and forums to ensure building partnerships and relationships to implement the NSS pathway effectively

Develop specialist protocols for use within the unit

To act as a role model and demonstrate resilience in leadership in contexts that are unfamiliar, complex and unpredictable;

To engage stakeholders and use high-level negotiating and influencing skills to develop and improve practice;

To work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems;

To support the NSS Pathway in the development of practices and roles that are appropriate to patient and service need through understanding the implications of and applying epidemiological, demographic, social, political and professional trends and developments;

To identify the need for change, proactively generate practice innovations and lead new practice and service redesign solutions to better meet the needs of patients and the service;

Ensure accurate, timely and accessible information and reports are produced for all areas of responsibility contributing to the performance-monitoring framework for the pathway. Identifying within these reports areas of high performance and where improvement is needed

Set up and co-ordinate nurse led telephone triage clinics,

Continually review, audit and implement improvements identified;

To ensure resource and cost effectiveness;

Manage the investigation of reported untoward incidents and cascade action plans / seek advice from the wider team. Identify areas of learning from these events and share the learning across the trust and wider areas;

To be proactively involved in developing strategies and undertaking activities that monitor and improve the quality of healthcare and the effectiveness of their own and others practice;

Continually evaluate and audit the practice of self and others at individual and systems levels, selecting and applying valid and reliable approaches and methods which are appropriate to needs and context, and acting on the findings;

Continually assess and monitor risk in their own and the practice of the wider nurse practitioner team and challenge others about wider risk factors;

To be actively involved with the NSS research agenda and act as investigator on relevant national research and audit programmes;

Critically appraise and synthesise the outcomes of relevant research, evaluations and audits undertaken and apply the information when seeking to improve practice;

Follow local governance systems when contributing to the development and implementation of evidence-based protocols, documentation processes, standards, policies and clinical guidelines relating to advanced and expanded practice;

To lead on standard setting and quality improvement through the use of audit within the team and making recommendations as a result of audit to be presented to the department and then overseeing any changes as agreed

Developing Self and Others

To develop and audit yearly team objectives reflecting the Local Delivery Plan;

Seize opportunities to generate and apply new knowledge to their own and others practice in structured ways which are capable of evaluation through the development of relevant expanded practice protocols for the team;

To provide both formal and informal educational training sessions for members of the team;

To encourage wider teams to be innovative in the development of patient care using evidence- based practice;

To be professionally accountable for own actions and the management of the NP team;

To be involved in and action patient satisfaction feedback mechanisms 360-degree feedback, staff and patient satisfaction surveys;

Actively seek and participate in peer review of their own practice and participate in annual appraisal, 360 feedback for self and team;

Demonstrate on-going professional development by keeping up to date with national and local developments in nursing / midwifery and NHS policy and practice;

To offer support and clinical supervision to nurses when required;

To promote the concept of work-based and inter-professional learning;

Develop external networks and represent the profession at national/international level in driving forward the nursing agenda at appropriate forums;

Demonstrates high-level communication skills and contribute to the wider development of those working in their area of practice by publicising and disseminating their work through presentations at conferences and articles in the professional press;

Maintains a professional portfolio of expanded and advanced practice.

2. Personal and Professional Development

Participate in the Trusts annual appraisal system designed to identify objectives and personal development opportunities.

Participate in the Trusts annual pay progression review (if applicable).

Ensure that all mandatory training is up to date

The post holder will be responsible for continually maintaining their working knowledge of local practices, policies and procedures and highlight ongoing training and development needs to their manager.
