Job summary
An excellent opportunity has arisen for a Physiotherapist or Occupational Therapist to join the Early Intervention Team for the position of Therapy Lead.
You will be a passionate leader in admission avoidance, early supported discharge within the Emergency Department or Same Day Emergency Day Centre (SDEC) and will work closely with acute Inpatient, Community and Primary partners to prevent unwanted admissions or improve flow / reduce length of stay for patients whom are admitted. You will provide support and leadership to the Therapy Team and will work alongside the Frailty Specialist and Discharge Planning Specialist Teams to provide excellent quality care driven by evidence based practices.
If successful, you will be required to demonstrate expert critical thinking in the clinical decision-making process, identify gaps within the service and assist with service improvement and development in line with national and local strategies. In addition, you will be responsible for facilitating effective teamwork and provide mentorship, teaching and support across the Early Intervention Therapy Team.
We are looking for someone who is passionate to help support the team through change and development. The team works 7 days with extended hours.
Main duties of the job
To work with the Service Lead and other Band 7 Clinical Lead Nurses to develop the service to best meet changing health and social care agendas by completing regular audits and data collection. To support the other EIT Clinical Leads in providing training and support for EIT Staff, Frailty Specialist nurses and DPSNs and Physiotherapists / Occupational Therapists in the team. To be responsible for providing clinical supervision and encourage professional development amongst the team of Therapists. To encourage Therapists and take a proactive, positive risk taking approach with advanced risk assessment skills in admission avoidance and discharge planning and support the junior Therapists to do the same.

Job responsibilities
The Early Intervention Team (EIT) is a proactive and innovative multidisciplinary team of Frailty Specialist Nurses, Discharge Planning Specialist Nurses, Occupational Therapists and Physiotherapists. The Early Intervention Team has been very well acknowledged for providing a proactive Admission Avoidance service, including Frailty screening, initiating comprehensive geriatric assessments for patients admitted to the Emergency Department, urgent treatment Centre and Medical Assessment Units.
