An exciting opportunity has arisen to join our Tyldesley and Atherton 0-19 team, working with the school nurses based at Tyldesley Health Centre. 

This vacancy is a permanent position, working 22.5 hours each week. You will be required to work flexibly between Monday-Friday, 08:00-17:00.

If you are considering a career in public health nursing and future application for the Specialist Practitioner health visiting or school nurse training, this is the position for you. We will provide you with a wealth of experience working with children, young people and families. 

You will work under the direction of the School Nurse – Specialist practitioner to deliver the 5-19 years Healthy Child Programme to include health screening/surveillance health assessment and a requirement to support the health needs of any vulnerable children/young people within the caseload. 

Pending or active NMC registration is essential. International applicants should note that there are alternate routes into the NHS.

Successful candidates will be expected to make multiple visits across the community daily whilst transporting the required clinical equipment. Cross borough working ensures that service delivery and equity within teams is maintained.

Main duties of the job

The role encompasses working within schools and communities, supporting parents/carers and promoting positive mental wellbeing in young people.

You will support delivery of the school aged Immunisation programme, attending high school sessions to Immunise as per the national schedule.

As a member of the School Nursing service you would have the opportunity to influence the future design of this role and the wider delivery to children and young people in Wigan.

Based in the Community division so you will need to be able to travel to Ashton, Wigan and Leigh. 

An Enhanced DBS with Children’s and Adults' Barred List checks will be obtained during pre-employment checks.

This post involves contact with patients face to face within a community setting. Therefore it cannot be ruled out that the postholder may come in to contact with people who have COVID-19, although the Trust provides PPE and takes all reasonable steps to minimise risk.

Detailed job description and main responsibilities

Duties and Responsibilities

To support delivery of the Healthy Child Programme 5-19 using evidence based practice to actively promote the health of children and young people. To be a named / link nurse for designated schools whilst holding a caseload which incorporates the 4 levels of intervention (Community, Universal, Universal Plus and Universal Partnership Plus). To deliver and engage in health promotion programmes and public health initiatives to prevent ill health, early identification of abnormalities and work with individuals, Schools and communities to reduce health inequalities as identified within individual School Health Plans. To conduct health needs assessments in a planned programme of care within a variety of settings. Offer appropriate interventions, delivering agreed programmes of care and evaluating the outcomes for the child young person and family. Plan and implement school entry assessments delegating duties as appropriate. Plan and implement health assessment at transition from primary to secondary school delegating duties as appropriate. Undertake comprehensive health assessments for identified children and young people, in line with the Healthy Child Programme 4-19 years of age. Establish clear and effective communication links with children and young people, parents/carers, education staff, and a range of other professionals from both voluntary and statutory organisations. Communicate outcomes of assessment and future care planning to the named School Nurse. Where necessary carry out home visits to children and families advising on all aspects of health as appropriate. Participate in health needs assessment of the local community, contributing to the planning, implementation and evaluation of strategies to address unmet health needs. To identify and assess children 5-19 with complex health needs, offering an enhanced integrated service within the multi-agency arena. To assess the needs of children and families following attendance at A&E, discharge from hospital working within trust guidelines. To identify, assess, report and work independently with oversight from the School Nurse to safeguard and protect the health and wellbeing of children and young people working within safeguarding policies and guidelines. To attend Child Protection case conferences and other meetings as appropriate. Listen appropriately and take into account any sensitive issues and communicate these to appropriate persons/agencies ensuring that children are protected and that disclosures are acted upon appropriately. Feedback the views and needs of children/young people, families and schools to the School Nurse as necessary, to enable appropriate interventions to be implemented. Ensure efficient and effective use of resources. Lead the planning, organisation and delivery of nurse led immunisation programmes in schools and other venues following patient group directions, actively promoting the National and Local Immunisation Programmes. To work in collaboration with other agencies to deliver services to children, young people and their families

Clinical Duties

Maintain active status on NMC register acting in accordance with the professional code and guiding documents in adherence with Revalidation Framework. Maintain contemporaneous, factual records in accordance with Trust guidelines and policies. Be aware of current working practices in accordance with Trust policies and procedures. Use their own professional judgement to identify solutions to a range of day-to-day problems, seeking advice where appropriate from the SCPHN Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines. Be responsible for own professional updating and keep abreast of current research based practice. Develop a working knowledge and experience of IT systems Observing the complaints and disciplinary procedures in accordance with the Trust policy. Contribute to clinical audit, research and clinical governance as part of the School Nursing Team. Participate in and complete the agreed induction programme for newly appointed staff Manage time with the team leader and team to ensure service is covered. Report sickness absence and return relevant forms to line manager. Maintain portfolio of professional development.

Leadership

Act to maintain an appropriate learning environment to meet the educational and developmental needs of student nurses and other learners as required Support other School Nurse Team member’s within the team, including the induction of new staff members. Ensure effective learning experience and opportunities to achieve learning outcomes for skill mix team. Develop and contribute to the development and implementation of policies, procedures and guidelines relevant to own area of work. Provide advice where services are developing.

Supervision and Development

The post holder will be able to recognise their own level of professional competence and identify sources of support as required. Provide mentorship, preceptorship and clinical supervision to staff as appropriate. Support the development of the team by teaching and assessing junior members of the team. Undertake mandatory and statutory training that support the post holder’s role. Undertake training specific to the post. Participate in individual performance reviews (PDR) and clinical supervision.

Communication

Communicate effectively with individuals, families and professionals in planning and delivering care. Utilise a range of communication methods with colleagues and managers to include electronic communication. Maintain effective working relationships, work effectively and support team members. Participate in formal and informal ways of communication both within the team and the wider primary care setting where appropriate. Work in partnership with school staff, health colleagues and other agencies e.g. Education and Social Services. Maintain contemporaneous and accurate Community NHS Trust records observing the Data Protection Act 1994, maintaining confidentiality, in accordance with NMC and Trust guidelines

Information Governance

Employees of the Trust must comply with the provisions of the Data Protection Act 1998. The postholder must not; either during the course of their employment, or following termination of their employment, disclose any information relating to service users or employees, or of the lawful business practices, of the organisation.

The postholder will be required, when and where appropriate to the role, to comply with the processing of requests under the Freedom of Information Act 2000

The postholder must comply with Trust policies that protect the information assets of the organisation from unauthorised disclosure, modification, destruction, inappropriate access or use and must pay due regard to the requirements of the NHS Confidentiality Code of Practice.

The postholder will be responsible for maintaining the clinical and or corporate records that fall within the remit of this role to the standards in Trust records management policies, and data quality processes and standards.

Training

The postholder must attend any training that is identified as mandatory to their role.

Responsibility for Health & Safety

Compliance with the Health & Safety at Work Act 1974 – the post holder is required to fulfil a proactive role towards the management of risk in all of their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards, and a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions.

Work Circumstances & on-call

Shift patterns are Monday to Friday 8 am to 5 pm, with occasional evenings. Flexible working patterns may be agreed with line manager.
