REPORTING TO: Home Manager
Purpose of Position
The role is of Care Home Assistant Practitioner (CHAP) will, after a period of extra training and competency assessment, provide clinical interventions to enhance the resident experience and the level of care service users receive under supervision of a Registered Nurse.
To support the Nurses to supervise and deliver excellent standards of care which promotes privacy and dignity, whilst enhancing independence, rehabilitation and personal choice.
To actively seek to empower service users to gain and maintain control over all aspects of their lives.
Recognise the service user's strengths, abilities and skills and seek to maximise their development and use.
Ensure meaningful involvement of service users is central to all activities, applying equal opportunities and anti-discriminatory policies and ensuring rights to privacy and confidentiality.
To support service users to meet their personal care needs in a way that respects the dignity of the individual.
To help in the care of the service users physical environment and in the general day to day activities of the Home.
To know and support the main aims and values of the Home and understand and implement the Home's policies and procedures.
To understand person-centred care and be passionate about the work you do.
Main Responsibilities
To support the Registered Nurse by providing patient-centred care. Please note that responsibility for the overall nursing care of the service user and clinical decision making remains the responsibility of the Registered Nurse.
To take responsibility under the supervision of the Registered Nurse for administering medicines, wound care, catheter care and other direct clinical tasks that you have been trained and assessed as competed to complete.
To take responsibility under the supervision of a Registered Nurse for blood sugar monitoring, tracheotomy care, PEG/PEJ, and undertaking nursing observations.
Take and record temperature, blood pressure, pulse and respiration rates, and food and fluid intake and output, as directed.
To clean, sterilise, store, prepare and issue dressing packs, treatment trays and other supplies.
Maintain patient stability by checking vital signs and weight; testing urine; recording intake and output information.
To help train junior members of staff in all aspects of their work in the Home.
To assist in meeting the personal needs of the service users in a way that respects the dignity of the individual and promotes independence, including outings, skill development
To help in the care of the service users physical environment and in the general day to day activities of the Home.
To carry out the Home's policies, practices and procedures.
To work, for part of the time, on a one-to-one basis with service users who may or may not have challenging behaviour.
To help the service users to lead as full a life as possible, to organise activities for them, e.g. handicrafts, games, reading, outings, etc.
To assist/ensure that the emotional, physical, medical and spiritual needs of the service users are recognised, assessed and met, as far as possible.
To encourage service users in taking decisions in matters which affect their lives.
To contribute to the assessment of need of each service user in conjunction with the service user, relevant professional agencies and, where appropriate, the service users' family.
To assist in developing and carrying out an appropriate Care Plan which provides a satisfactory quality of life for that person, carrying out that programme and recording any appropriate information relating to same. To report to senior staff any action/recommendations which have not been carried out.
To assist in carrying out risk assessments appertaining to individuals and activities, and to ensure that all risk assessment proposals are carried out.
To assist the Home Manager with supervisions and appraisals of care staff.
To take responsibility for Health & Safety issues, e.g. reporting any dangers to self and others, etc.
To help service users with mobility problems and other physical disabilities including incontinence. Assisting with use and care of aids and personal equipment.
To answer door and telephone and greet visitors in an appropriate manner.
To assist professionals, e.g. GP, nurse, physiotherapist, chiropodist or any other external professionals if necessary.
To read and write reports, taking part in staff and service users meetings and in training activities as directed, on or off site.
To read all relevant communications, sign, and carry out instructions.
To report any changes in service users' condition to the nurse in charge and Management team.
To record incidents involving service users, staff or visitors in the Accident Book, or other relevant documentation.
To record all complaints, comments, etc. appropriately and take proper action where possible, and report the matter immediately to management.
To care for service users' property and that belonging to the Home as directed.
To care for equipment and report any defects to the Manager.
To clean up spillage's as they occur using appropriate equipment supplied and as specified in the Control of Infection Policy/COSHH.
To adhere to all aspects relating to confidentiality and not discuss the Company, service users, or other information which is acquired via your role, with others not connected to or who have no right to the information.
To report any situations/concerns immediately to management.
To be flexible and work to the benefit of service users, and to assist with any other duties if required.
To ensure the provision of health care arrangements, which includes ordering, recording and administration of medication.
To support within other services across the group according to the needs of the business.
To be flexible to travel between services across the group for support purposes.

NVQ Level 3 Care or equivalent, completion of an accredited CHAP's Programme.
At least 12 months experience in the sector
Working with people with a disability or special needs or older people

Medicine Administration
Previous experience with patients with tracheotomies
Previous experience with service users who have PEG/PEJ fitted
Phlebotomy
Understanding of what MCA and DoLs are and how this relates to practice
Ability to carry out planning and how to review these.
