Care Home Assistant Practitioner Job Description

Job Summary:

We are seeking a highly skilled and compassionate Care Home Assistant Practitioner to join our team at Careline Lifestyles. As a Care Home Assistant Practitioner, you will play a vital role in providing high-quality care and support to our residents, working closely with our nursing team to deliver exceptional standards of care.

Main Responsibilities:

  • Provide clinical interventions to enhance the resident experience and the level of care service users receive under supervision of a Registered Nurse.
  • Support the Nurses to supervise and deliver excellent standards of care which promotes privacy and dignity, whilst enhancing independence, rehabilitation and personal choice.
  • Recognise the service user's strengths, abilities and skills and seek to maximise their development and use.
  • Ensure meaningful involvement of service users is central to all activities, applying equal opportunities and anti-discriminatory policies and ensuring rights to privacy and confidentiality.
  • Support service users to meet their personal care needs in a way that respects the dignity of the individual.
  • Assist in the care of the service users physical environment and in the general day to day activities of the Home.
  • Support the Registered Nurse by providing patient-centred care.
  • Take responsibility under the supervision of the Registered Nurse for administering medicines, wound care, catheter care and other direct clinical tasks that you have been trained and assessed as competent to complete.
  • Take responsibility under the supervision of a Registered Nurse for blood sugar monitoring, tracheotomy care, PEG/PEJ, and undertaking nursing observations.
  • Carry out regular nursing observations and understand the implications of the results of your findings.
  • Act as a resource to other care staff.
  • Advocate for client choice and independence.
  • Provide personal care and hygiene duties for people with a disability.
  • Work on own initiative and as part of a team.
  • Organisational skills.
  • Strong interpersonal skills (listening, verbal and written communication).
  • Maintain high standard of household duties and cleanliness.
  • Cookery skills (basic).

Requirements:

  • MUST have CHAP training already & NVQ Level 3 Care or equivalent, completion of an accredited CHAP's Programme.
  • At least 12 months experience in the sector.
  • Working with people with a disability or special needs or older people.
  • Staff/shift supervision.
  • Medicine Administration.
  • Previous experience with patients with tracheotomies.
  • Previous experience with service users who have PEG/PEJ fitted.
  • Phlebotomy.
  • Understanding of what MCA and DoLs are and how this relates to practice.
  • Ability to carry out planning and how to review these.

Working Hours:

This is a night shift position.
