Job summary

CareCoordinators are part of a GP surgerys multidisciplinary team, workingalongside GPs, social prescribing link workers (SPLWs), pharmacy staff,nurses, surgery administrative staff etc. to provide and coordinate, anall-encompassing approach to personalised care and promoting and embedding thepersonalised care approach across the Primary Care Network

Main duties of the job

- working closely with clinical and admin team

- patient contact (Face to Face and telephone)

- Administrate Task and Duties

- patient recall for health checks, QOF, Vaccinations

- Social media overview

- running EMIS reports, creating EMIS Reports

- Batch messages via ACCURX

- Reception Desk Support

Job responsibilities

We are seeking an enthusiastic and committed carecoordinator to join our friendly team providing high quality care to ourpatients. A pleasant manner, attention to detail, experience of people facingenvironments are essential. As a care coordinator, the successful applicantwill work closely with the clinical and reception team. Knowledge in EMIS and Docman is desireble but not essential.
