Job summary

St Pauls Primary Care Networkpresents an exciting opportunity for a part time experienced ClinicalPharmacist to work across the four practices that form the network, all housedin St Pauls Medical Centre in the heart of Cheltenham, Gloucestershire, in theSouth West (Weston House Practice, St Catherines Surgery, St Georges Surgery and The Royal Well Surgery).

The successful candidate willwork across the four practices as part of the Pharmacy Team closelysupported by the GPs, existing Clinical Pharmacists and the wider clinicalteam.

Main duties of the job

The role will provide a mix offace to face and virtual patient centred consultations and telephone support topatients and external agencies. You will be expected to use your clinicalskills to carry out a variety of tasks including prescribing within yourcompetency, medication reviews; reconcile medications at hospital discharge andclinical letters; oversee complex needs of house bound and care home patientsand look to review and de-prescribe in polypharmacy.

Ideally suited to anIndependent Prescriber with experience in primary care, you will be expected tosupport less experienced pharmacists and technicians and encouraged to managesuitable long term conditions and use your skills within the clinical multidisciplinaryteam to advise on medicine optimisation, safety and appropriate prescribing inline with local and national policy.

Job Purpose

To work across the four St Pauls practices within St Pauls Medical Centre, Cheltenham ensuring that medicines management processes within these practices are optimised to achieve highly effective, safe and patient centred prescribing. To work closely with individual practice leads to tailor the delivery of network priorities and targets with a practice-based focus. As an independent prescriber, to undertake face to face and telephone-based patient consultations, reviewing patients with potential medication issues and long term conditions. This will include de-prescribing within the context of polypharmacy and frailty. To offer professional leadership and facilitate shared learning with other less experienced clinical practice pharmacists and pharmacy technicians working in associated local practices and networks to support the development of their role.

Responsibilitiesunderpinning the role

Toenrol or be enrolled in or have qualified from an accredited training pathwaythat equips the post holder to be able to practice and prescribe safely andeffectively in a primary care setting and in order to deliver the keyresponsibilities of the role. Currently, the Clinical Pharmacist trainingpathway: Primary CarePharmacy Education Pathway. (PCPEP)

Tobe an Independent Prescriber or close to completion and beable to undertake face to face consultations with patients.

Toreceive support and supervision from a Senior Clinical Pharmacist and GP lead toallow the post holder to develop and do the job safely and confidently.

Toplan and organise the post holders own workload, including audit and projectwork, and training sessions for members of the medicines management team,practice team, community pharmacy team, community nurse team, patients, carers,etc.

Toattend local, regional and national meetings of relevance some of which will bedefined within the national PCPEP for this role supported by CPPE, ManchesterUniversity.

Todevelop and facilitate a good working relationship with community pharmacists,hospital pharmacists and other local providers of healthcare.

To becomefamiliar with System1 GP system and supporting IT and to record personallygenerated information and maintain a database of information relating to thework done in the practice(s).

Personallygenerated information includes information and records relating to audit andclinical work undertaken by the post holder, reference notes relating toclinical/technical information, etc.

Thedatabase includes maintaining up-to-date, detailed records of all work done inthe practices for which the post holder is accountable (done by the post holderor others) using appropriate coding.

Toundertake any other duties commensurate with the post holders grade as agreedwith the Network/ practice lead GPs or the post holders line manager,including mentorship of members of the Pharmacy Team. .

Tomaintain registration as a pharmacist with GPhC and comply with appropriateprofessional codes and standards.

Asappropriate to the post, to maintain and develop professional competence andexpertise, keep up to date with medical/therapeutic evidence and opinion, andlocal and national service, legislation and policy developments, agreeobjectives and a personal development plan and participate in the appraisalprocess. Some of this will be supported through the PCPEP Pathway.
