We are looking to appoint a new enthusiastic and motivated Senior Community Nurse to join the Airedale Collaborative Care Team (ACCT).
This is an exciting opportunity for an enthusiastic and motivated nurse with relevant experience to join our established integrated intermediate care team providing and Urgent Community Response and Virtual Ward. The Collaborative Care Teams have continued to develop and the CQC reported our team is described as an outstanding example of community team are passionate about our service and take real pride in putting our patients at the heart of all we do, this is reflected in our collaborative approach to teamwork. The Collaborative Care Teams include registered nurses, physiotherapists, occupational therapists, mental health nurses, community advanced practitioners, community support workers and assistant practitioners. To support our multi-disciplinary approach we actively support skill sharing with all staff working within a Competency Based offer a robust induction programme, on the job training, annual appraisal, 1:1 supervision and opportunities for development. Main duties of the job
The post holder will be a registered Nurse responsible for ensuring the highest standard of clinical care is provided for patients, and is required to work flexibly to meet the needs of the service across Community Intermediate Care Services.
To deliver high quality, personalised care to individuals requiring an urgent community response to avoid unnecessary hospital admissions and a virtual ward to support patients to remain at home wherever possible and facilitate safe hospital discharge. The post will require individuals to work in the community in collaboration with health, social and voluntary services to support people who are in crisis in their own home. The team support early discharge from hospital providing Virtual Ward to people who require a period of rehabilitation to remain safe in their home. Senior nurses are expected to co-ordinate the service and manage the daily team on duty, they will support staff to respond efficiently and effectively to people needing a multidisciplinary team response to assess their needs and ensure the team have the correct skills to deliver clinical intervention safely. The successful candidate will be required to support the development of the team and provide a consistently high standard of care to patients.
About us
The service receives referrals from a wide range of health and social care professionals, with the aim of supporting patients with a health related crisis at home. The goal is to avoid an acute hospital admission and/or support early discharge. We also provide support to patients who require an out of hours district nursing service and provide input to people requiring intermediate care beds in the community.
Collaborative Care Teams provide support to patients within their own homes that require IV therapy, this includes clinical assessment of patients, management and escalation of unwell patients, patient centred care planning, administration of IV medication, peripheral cannulation and on-going care of cannulas, venepuncture and management of Central Venous Access Devices.

The post holder will support the Team Leader in the provision of strong clinical leadership and ensuring the principles clinical leadership are implemented at a local level within the designated team. The post holder must adhere at all times to The Code: Professional standards of practice and behaviour as published by the Nursing and Midwifery Council, they must work within the policies and guidelines as laid down by ANHST

GENERAL DUTIES:
CLINICAL
Provide quality clinical assessment and reassessment of complex needs and plan care to ensure individual patient care needs are met. Lead the standard of nursing care and treatment for patients with acute and long term complex health needs. Administer medications to patients consistent with protocols, standards and legislation. Act as a positive clinical role model through demonstration of expert practice. Lead by example to motivate and empower others. Supervise team members in the development of programmes of induction and preceptorship to ensure safe practice. Develop own skills and knowledge, including active participation in clinical supervision, ensuring provision to other staff within the team. Participate in the development of support worker roles to meet the care needs of patients. Ensure completion of patient records in accordance with NMC guidelines for record keeping ensuring a quality service is delivered. Comply with the requirements of the Caldicott report, the Data Protection Act, Access to Health Records and subsequent legislation issued in relation to confidential data. Ensure staff practice in accordance with these guidelines. Maintain awareness of care developments to enable the continued provision of quality care. Participate in clinical bench marking, ensuring examples of good practice are shared.
MANAGEMENT/LEADERSHIP RESPONSIBILITIES
Take responsibility in the absence of the Team Leader, meeting shortfalls in staffing, delivering effective care and effectively managing risk. Support the delivery of a quality service contributing to the development and evaluation of standards of patient care, working with the wider multi-disciplinary team. Monitoring and reporting audit results Ensure developments are supported in practice through the implementation of policies, protocols and clinical guidelines within the team. Address the concerns of patients and service users and respond to their suggestions for local quality improvements. Support the development of the multi-disciplinary team, ensuring effective working relationships and communication is maintained. Maintain a safe working environment, being aware of the necessity to comply with Health and Safety regulations, ensuring risk incidents/assessments are documented and reported in accordance with Trust procedures. Liaise effectively with Community Service Delivery Team Leader on site issues. Ensure equipment held is maintained according to manufacturers specifications and necessary repairs are undertaken. Support the Team Leader in ensuring staff are trained and supervised where appropriate in the correct use of equipment. Contribute to the recruitment and selection of staff and the organisation of their induction/orientation programme. Deputise for the Team Leader where appropriate.
EDUCATION AND TRAINING
Develop own clinical and professional skills through identification of performance objectives with the Team Leader and engage in the continuing personal development planning. Participate in performance review and appraisal. Promote a positive learning environment. Facilitate and participate actively in teaching programmes for qualified staff, students and multi-disciplinary team. Support the development of students and staff through participation in teaching programmes and supporting individuals at work promoting a culture of continuous professional development through life long learning. Assessment of individuals learning needs through staff appraisal and development training. Promote evidence based practice. Support and participate in research, audit and project work, alerting others to findings and complications.
