About the Role

We are seeking a highly skilled and motivated Senior Community Nurse to join our Airedale Collaborative Care Team (ACCT). As a key member of our team, you will be responsible for delivering high-quality, patient-centered care to individuals requiring an urgent community response and virtual ward services.

Key Responsibilities
  • Provide clinical leadership and supervision to a team of community nurses, ensuring the highest standard of care is delivered to patients.
  • Develop and implement care plans that meet the individual needs of patients, working collaboratively with other healthcare professionals and community services.
  • Conduct comprehensive assessments and reassessments of patients, identifying their needs and developing strategies to meet those needs.
  • Administer medications and treatments as prescribed, ensuring patient safety and well-being.
  • Lead by example, promoting a positive and supportive team culture that encourages staff to develop their skills and knowledge.
  • Participate in the development and implementation of policies, protocols, and clinical guidelines, ensuring they are evidence-based and effective.
  • Contribute to the recruitment and selection of staff, ensuring the team has the necessary skills and expertise to deliver high-quality care.
  • Deputise for the Team Leader as required, taking responsibility for the team's workload and ensuring continuity of care.
Requirements
  • NMC registration as a Registered Nurse (Level 1)
  • Teaching and assessing qualification (e.g., ENB998 or equivalent)
  • Post-registration continuing professional development (CPD) graduate
  • Relevant post-registration experience, including leadership and management roles
  • Ability to work autonomously and as part of a team, with excellent communication and interpersonal skills
  • Full clean driving license and access to a car
