Job summary

An exciting opportunity has arisen to join HerefordshireCommunity Learning Disability Teams as the Band 7 Community Learning Disability Clinical LeadNurse , Full Time hours ( 30 hours will be considered) based at Liverpool Court, Commercial Road, Hereford.

The successfulapplicant will lead a small but tenacious and committed team who are passionatein ensuring that individuals with a learning disability receive equitablehealth care alongside managing a clinical caseload and maintainingregular visits for service users and their carers.

This role ischallenging, varied and interesting and would include providing clinicalleadership and supervision to the team. The team are currently involved inexciting service developments including RcPsych QNLD Accreditation ,STOMP,Dementia pathway, transitions pathways to better meet the needs of individualswith learning disabilities. We welcome creativity, enthusiasm, and passion witha go for it attitude to best meet patients health needs.

You willreceive regular supervision, on-going CPD & training, and support withinthis role to help further develop your skills and competenciesin this specialist field.

Main duties of the job

Work within available resources toensure consistent, safe and timely service delivery including processing andallocation of referrals, access to assessment, day to day coordination ofactivity.

Provide regular managerial supervisionand support to health staff within the CLDT, and clinical supervision asappropriate.

Carry out staff appraisals, inaccordance with agreed policy and to identify the learning and developmentneeds of staff, ensuring registration is kept up to date and in order.

Assist the CLDT Manager in ensuring thatthe service achieves its aims in accordance with the organisation's performanceagenda and business plan.

For people on their caseload, beresponsible for the whole process of assessment, care planning, monitoring, andreview.

Act as a senior/experienced member ofthe CLDT in relation to risk assessment giving advice and direction asrequired.

Manage inspections and audit of the teamif required.

Provide clinical/management supervisionas appropriate to health staff within the CLDT.

Facilitate open communication amongststaff within the CLDT as well as with other teams and services with the aim ofpromoting multi-disciplinary approaches to care and integrated team working.

Work with the Area Team Manager to deliver a service which issoundly evidence based, has a reputation for quality and ensures that allavailable resources are utilised and within budget.

Provide a consultative role within the CLDT in relation tohealth issues.

Be expected to work flexibly in a joint managerial andclinical role.
