An exciting opportunity has arisen for a compassionate and innovative individual to work as an Advanced Care Practitioner (ANP) within the Haemoglobinopathy Service. The ACP will play an integral role in the management and delivery of high-quality patient care who can act autonomously, within their professional boundaries. The post holder will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment, and evaluation of their care. They will demonstrate safe clinical decision making and expert care for patients as an integral part of a dedicated. multidisciplinary team. The post holder will incorporate the Associate Care Practitioner role dimensions of expert clinical practice, education, leadership, management, and research This post is full time 37.5 hours per week.

Main duties of the job

To liaise with all key members of the multidisciplinary team to advise and support colleagues in the delivery of expert patient care across professional boundaries. To independently manage a patient caseload utilizing advanced decision making and extended clinical skills. To undertake Haemoglobinopathy practitioner led clinics and to be responsible for individualized care planning and contribute to annual reviews. To work autonomously to deliver specialist Haemoglobinopathy clinical care appropriate to the needs of the patient’s group and based on principles that are research based and which enhance the quality of patient care. To co-design the planning, delivery, and evaluation of care pathways for patients with haematological conditions. To lead on training and education within Haemoglobinopathy specialist areas such as outpatient clinics the Haemoglobinopathy day unit and in patient wards. To promote an innovative and progressive attitude to the continual improvement of patient care through reviewing the evidence base and embedding this in practice through engagement in research. To act as a visible Haemoglobinopathy role model and expert.

Detailed job description and main responsibilities

Maintain a professional behaviour at all times and promote a positive image of North Middlesex University Hospital Trust (NMUH) at all times, in line with organisational values. Ensure that the needs of the Haemoglobinopathy patients are placed at the centre of care delivery, thus achieving trust and national standards for clinical quality, safety and patient experience. To manage a caseload of patients within practitioner led clinics developing individualized care plans. Demonstrate expertise in clinical examination, clinical interpretation of results and diagnosis ensuring patients are treated appropriately according to national guidance. Provide professional, clinical, and managerial leadership to nursing and associated staff, and ensure the provision of a high quality, responsive Haemoglobinopathy service. Support Emergency care pathways providing expertise and support in managing patients according to national guidance. Conduct ward rounds both independently and supporting medical staff as required. To coordinate and deliver specialist interventional procedures for Haemoglobinopathy patients. Be responsive to the needs of inpatients, reviewing care plans and prescribing required care and treatment regimens within scope of practice. Act as an expert resource across the organisation.

•Registered Nurse/Practitioner •Post registration qualification in related speciality •Teaching qualification or evidence of ability to teach. •Educated to degree level. • Able to demonstrate significant. •experience in speciality

Skills and abilities

•Knowledge of current clinical developments within speciality •Delivery of red cell exchange both manual and automated • Phlebotomy/cannulation • Midline/femoral line insertion • Intravenous drugs administration •Management of PCA •Ability to work alone and to prioritise workload. • Knowledge of being accountable practitioner •IT skills and knowledge •Punctuality and reliability in attending to duties. •Skilled practitioner who will act as a role models. •Skilled in verbal and written communication •Experience of teaching colleagues and patients using appropriate terminology
