An exciting opportunity has risen for a Band 6 to 7 (run through, dependent on achieving competencies) Community Adult Haemoglobinopathies Nurse Specialist to work in conjunction with teams across NCL (North Central London) in delivering a high-quality specialist sickle cell and thalassemia service in the community. This is a very new post in an ever-expanding team.

We are looking for dynamic, compassionate leaders to support the care of patients with red cell disorders in North Central London working largely in a community capacity.

The chosen individuals will be proactive in making regular contact with all patients on their caseload and will intervene more frequently with those who are admitted or being seen in clinic. This role is pivotal in making a difference in the lives of patients with red cell disease and the post holder will liaise as need be with other health and care practitioners. Importantly, they will also work closely with voluntary sector and local authority to address all aspects of health and wellbeing and focus on preventing crises as far as possible.

You will work with a future focused team across NCL and whilst linked to individual centres, will be agnostic in providing care when needed to any NCL patient.

Main duties of the job

In conjunction with the haemoglobinopathies teams across North Central London, the post holder will be responsible for the delivery and ongoing development of a high-quality specialist sickle cell and thalassemia service in the community. The post holder will provide clinical leadership and practice at an advanced level demonstrating a high level of autonomy and clinical decision making in the community.

Key deliverables will include:
  • Provision of health education and guidance on self-management to prevent exacerbation of patient's condition to reduce incidences of acute episodes requiring admission to secondary care
  • Enhancing compliance with preventative actions such as vaccinations and prophylactic medications
  • Addressing social and lifestyle factors which impact on condition management and overall quality of life and directing to appropriate local resources
  • Building trust with patients.
  • Targeted support to high-intensity users of secondary care including directing patients into existing NCL programmes for frequent ED attenders
  • In-reach service to secondary care.
  • Developing close links with multidisciplinary colleagues in primary, secondary, and social care to enhance continuity of care for patients.
  • To enable implementation of individual care plans for patients in all settings
  • Implementation of a patient outreach health education group programme, and organisation of patient led education session.

  • Postholder will work across the whole of North Central London but will predominantly be based at the Whittington or North Middlesex University Hospital (Inner London Weighting).
  • To act as an expert resource for patients and their families, giving advice, education and support that is appropriately planned to meet each individual's needs empowering them to manage their treatment regimes, to recognise any deterioration or acute episodes requiring immediate specialist attention and how to access the necessary support.
  • North Central London (NCL) comprises of the boroughs Camden, Islington, Haringey, Islington, and Barnet. Within NCL, there are 3 Specialist Haemoglobinopathy Teams (SHTs), University College London Hospitals, North Middlesex University Hospital, and Whittington Health NHS Trust.

    The Red Cell Network (TRCN), is commissioned by NHS England and led by University College London Hospitals, holding responsibility as the HCC for NCL to support the provision of specialist and non-specialist haemoglobinopathy services to adults and children with sickle cell disorder, thalassemia, and other rare anaemias. Close working with inpatient, outpatient and community teams across the network is expected.

Qualifications & Knowledge

Essential

  • First level Registered Adult Nurse (with the NMC)
  • Relevant educational/ training qualification/certificate/experience
  • Leadership or mentorship course
  • Demonstrable theoretical and practical knowledge of Haemoglobinopathy

Desirable

  • Masters level education or equivalent experience
  • Qualified non-medical prescriber

Experience

Essential

  • Experience at band 6 or above in a related speciality
  • Experience managing patients with sickle cell disease and thalassaemia
  • Experience of acute sickle cell crisis management
  • Experience in pain management
  • Evidence of management of change, clinical leadership, and development of services
  • Experience of working in the community
  • Experience participating in audits and clinical research
