This post is based primarily at Savernake Hospital in Marlborough, wealso have inpatient wards at Warminster Community Hospital and ChippenhamCommunity Hospital. There is some requirement within the post to flexibly crosscover other teams, this is to enhance our team working across inpatientservices and to ensure that we all have a broad range of clinical will be working alongside a clinical team comprising of ConsultantPractitioners/Associate Specialists/GP trainees and other ACPs supporting theward teams with the medical aspects of patient care while developing your ownclinical expertise within the specialties of Frailty, Older Peoples Medicine,Rehabilitation & End of Life Care.

Applications are invited from candidates who are Registered Nursing or Physiotherapycolleagues, holding an MSc in Advanced Clinical Practice (or working towards)and have relevant post registration experience or those people willing toundertake an MSc and who meet the academic criteria to do so. We have limitedthis post to Nursing and Physiotherapy only due to current prescribingrestrictions for other professions.

Main duties of the job

Experience at Band 7 with knowledge of effective change management skills and a proven track record in the delivery of high-quality clinical care are essential.

The successful candidate will be a team player who is flexible and adaptable to the needs of the people who access our services.

The post is banded at 8a (AFC) on completion of the MSc in Advanced Practice, or at band 7 (dependent on stage of training/modules ratified).You will be expected to work across all four Advanced Practice Pillars.

Clinically working independently at an advanced level in the directdelivery of high-quality care making critical clinical decisions based on variedskills and assessment techniques.

Provide clinical leadership and support across the entire inpatient MDTand to support the Senior ACP Leadership Team in developing the ACP service toimprove the patient experience.

Supporting colleagues to achieve training and development requirements.Ensuring that you are maintaining your own portfolio of learning in accordancewith our organisational ACP policy and HEE guidance.

Ensure consistent high-quality care through audit and research programs.

Informal visits are recommended and welcomed.

Qualifications

Essential

Registered with the NMC or HCPC First level nursing or healthcare degree Recognised teaching or mentorship qualification equivalent to level 3 Independent Non-Medical Prescriber Qualification MSc in Advanced Clinical Practice or Advanced Nurse Practitioner

Additional Criteria

Essential

Access to a vehicle that can be used for business purposes and hold a full and valid driving licence.

Experience

Essential

Previous experience in gerontology or a relevant medical speciality at band 6 above. Evidence in challenging clinical practice and leading in the development of new pathways of care and delivering best practice Evidence of successfully resolving complex problems. Experience in carrying out clinical audit and risk assessment Proven skills in leading, managing and developing teams Evidence of successful resource management.

Desirable

Experience as an Advanced Nurse Practitioner/Advanced Clinical Practitioner Strong IT Skills including excel and power-point Knowledge of national NHS drivers and initiatives
