Job summary

An opportunity has arisen for an individual to join the Community Integrated Learning Disability Team .

The post is hours per week.

The team offers an integrated specialist health and social care service to people with a learning disability and their carers, living in community settings. You will carry a caseload and work under the supervision of a registered learning disability nurse to support service users with complex health needs. You will have experience of completing health assessments and person centred health actions plans and will be able to facilitate and support ongoing access to healthcare . You will support people with a Learning Disability to access annual health checks and complete pre- health assessments. You will be involved in health promotion activities as directed by the Senior Team are looking for enthusiastic and motivated individuals who have excellent values, interpersonal and communication skills. You should be innovative, person centred and committed to improving outcomes for people with a learning disability across health, social care and third sector services.

Main duties of the job

Responsible for following treatment plans set for individual patients by therapy and nursing staff. Carry out all interventions competently and to a high standard, as delegated by the senior integrated team in line with own scope of practice. Take responsibility for maintaining and developing own clinical competency within the workplace. Take an active role in follow-up of patients, monitoring progress against the personalised care plan and reporting back to the designated To undertake a range of duties which involve direct patient care, under supervision or without supervision following competency assessment. Assist in the monitoring of patients, by undertaking and accurately recording and reporting all interventions. Utilise specialist tools to support assessment Be involved in Health promotion activities, as directed by the senior team. Education and Qualifications

Essential

Foundation Degree in Health and Social care-long term conditions pathway Experience of working in more than one clinical speciality across health and/or social care Basic IT skills
