Job responsibilities

The Lead Practice Nurse will beresponsible for the management of the practice nursing team, consisting ofnurses and HCAs.

The Lead Practice Nurse will beresponsible for several clinical areas such as infection prevention and control, healthpromotion, chronic disease management, health prevention, well women and wellman clinics, as well as supporting the practice managementteam in the reviewing and delivery of clinical policy and procedure.

The Lead Practice Nurse will beresponsible for the implementation of processes for the effective management ofpatients with long-term conditions using evidence-based practice including carefor elderly and housebound patients.

Working as a senior member of thepractice multidisciplinary team, the post holder will ensure nursing servicesare delivered effectively to the entitled patient population.

Northfield Surgery, FieldRoad Surgery and Mayflower Medical Practice must ensure that the post holderhas access to appropriate clinical supervision and an appropriate namedindividual in the organisation to provide general advice and support on aday-to-day basis.
