Were currently looking for someone to complement our experienced Primary Care Team, delivering physical healthcare to at risk and highly complex young people. Prison is a community and, as nurses, we are part of a team working in that community; we are a cog in the bigger machine and provide a service to those living here.

An informal visit is highly recommended.

Main duties of the job

As a Clinical Team Leader you will be required to: Complete initial health assessments Triage and assess minor illnesses and injuries and help manage long term conditions. Provide emergency response in keeping with the community model. Support with and deliver childhood immunisation and vaccinations. Co-ordinate the shift ensuring a safe, caring, effective and well-led service is provided 24/7. Liaise with the senior management team and undertake line management responsibilities. As well as clinical responsibilities, you will also perform some admin & clerical duties such as assessing, planning, implementing and evaluating care plans, audits, Datix and link nurse role work streams. You will lead by example with LCHs vision, values & behaviours. HMWYOI has excellent onsite facilities, with free parking, two staff gyms and a coffee shop. It is situated very close to the A1(M) with links to Leeds, Harrogate and York which makes commuting easier.

Job responsibilities

Job Purpose

The post holder will beresponsible for their own caseload within their speciality area as anautonomous practitioner. In doing so, they will assess, plan, deliver andevaluate patient centred programmes of care ensuring that they practice withintheir sphere of competence and knowledge. The post holder will work toprofessional and regulatory body codes, standards and guidance at all timesensuring that their practice is grounded in evidence based theoretical andpractical knowledge. The post holder will provide the day to day linemanagement of a team of clinicians.

Key Responsibilities

1. Clinical

Maintain clinical credibility demonstrated throughundertaking clinical practice at a frequency agreed with ServiceManager/Team Manager/Senior Practitioner Pathway Lead in the relevant field.

Practices as an autonomous clinician to supportservice delivery, responding to social, scientific, clinical and ethical issuesin line with the values and agreed model within the service

Works to standards of proficiency identified bytheir professional and regulatory bodies

Tocomply with the organisations Infection Prevention and Control requirements,including bare below the elbows dress code for staff with a direct caredelivery role

Experience

Essential

Post-registration experience in the clinical field or a closely related clinical area. Working as part of a wider multidisciplinary team as well as working autonomously having the ability to make complex and critical judgments. Communication skills Management of workload and prioritisation and supporting junior staff. Displays care, compassion, sensitivity and responsiveness to other peoples feelings and needs. Able to work as part of a team, co-operating to work together and in conjunction with others and willing to help and assist wherever possible and appropriate appreciating the value of diversity in the workplace. Able to develop, establish and maintain positive relationships with others both internal and external to the organisation and with patients and their carers. Able to work under pressure, dealing with peaks and troughs in workload. Positive and flexible attitude to dealing with change; able to respond to the changing needs of the patient in an appropriate and timely manner; willing to change and accept change and to explore new ways of doing things and approaches. Highly motivated, reliable and resilient. Reflective practice skills. Has a strong degree of personal integrity; able to adhere to standards of conduct based on a culture of equality and fairness.

Desirable

Working in hospital, community or primary care based teams. Previous experience in a secure / prison environment.

Qualifications

Essential

Professional degree or diploma in either Adult or Pediatric Nursing or equivalent HCPC registration Appropriate statutory professional body registration NMC. . Must be willing to participate in any relevant training identified to develop skills required to carry out duties. Maintains a portfolio of CPD in line with regulatory body standards and actively working towards revalidation.

Desirable

Mentorship or Practice Assessor Course or equivalent ILS / PIL.

Practical & Intellectual Skills

Essential

Assessment, planning, implementation and evaluation skills. Performs test / procedures / diagnostics relevant to clinical role proficiently. Recognising deteriorating patient and escalate appropriately as per local guidance. Workload management including delegation of tasks. Competent IT skills word processing, email in order to maintain electronic patient records. Effective verbal, non-verbal and written communication skills including communicating complex or potentially distressing information to patients / carers. Risk assessment skills.

Desirable

Experience in research and/or clinical audit. Immediate Life Support / Paediatric Immediate Life Support training.

Skills and Attributes

Essential

Experience and passion in the following: Time management Change management / clinical developments Clinical governance Health Promotion. Theoretical and clinical knowledge appropriate to service area and profession. Knowledge of relevant current issues in health and social care and Health and Justice. Knowledge of NMC Code of Professional Conduct. Demonstrates awareness of limits to knowledge base. Basic understanding of clinical governance and quality measures. Ability to challenge appropriately and advocate for the needs of the patient and service.

Desirable

Auditing and research NHS policies and political awareness. Awareness of LCH Trust 3 Values, 1 Vision and 7 Magnificent behaviours. Have knowledge and understanding of childrens safeguarding & child protection. Basic Mental Health Awareness.
