Taunton and West Somerset Community Rehab service is looking for a Band 6 Occupational Therapist to join our team, delivering goal directed community rehab programmes to people in their own homes and/or local communities. This post is based in Taunton but there may be times when there will be a requirement to provide cover in West Somerset. Working alongside OTs, Physiotherapists, Rehab Assistants/Practitioners and Pharmacy Tech, you will be facilitating rehabilitation to patients in the community. You will hold a caseload of patients with complex rehab needs, working alongside team members, family, carers and other services involved in the current episode of care. You will be supporting by Clinical Specialist OT & PT and Therapy Team Lead and participate in service development and peer support.

This post is being advertised for 30 hours but some negotiation may be considered.

Main duties of the job

The postholder will be required to use advanced assessment, risk assessment and clinical reasoning skills to identify achievable outcomes of treatment and to recommend the best course of intervention, developing comprehensive treatment plans, in conjunction with other professions and agencies. Using these skills, the postholder will participate in the weekly Duty rota triaging and prioritising incoming referrals.

The postholder will be committed to a person-centred approach to your work, have an ability to prioritise and work flexibly.

The postholder will be able to appropriately delegate work to the Rehabilitation Assistants.

The postholder will act as a Clinical Supervisor for Band 3/4/5 staff members and students.

The postholder will take an active role in the development of the Community Rehabilitation Teams within Somerset NHS Foundation Trust and their own continuing Professional development.

Qualifications

Essential

Diploma or degree in Occupational Therapy. HCPC registrations. Evidence of CPD maintained in a portfolio including attendance at a recent post graduate courses Car driver with relevant business insurance Evidence of a range of community and clinical rehabilitation areas of practice including elderly patients with multiple complex pathologies/neurological and orthopaedics etc.
