Job summary

You would play a significant role in the patient care. This will includeproviding support for patients and families who are making decisions from diagnosisand throughout their pathway; wherever possible responding to patients holistic needs.

As asenior nurse working independently but within a team environment, you will needto be a confident, outgoing person with excellent communication skills and demonstrate an ability to be highly self-motivated.

The successful candidate willneed to be passionate about supporting the team to focus on servicedevelopment, auditing outcomes and improving our patient pathways. A willingness to participate in professional development anddevelop further nurse led services are highly desirable. In-house training andeducation will be provided to develop and build on an existing knowledge base.

This post may also be offered as a development position starting at a Band 6, to an appropriate candidate.

Main duties of the job

To provide an effective and high quality patient-focused service that is sensitive to the needs of users and providers.

To provide clinical, social and psychological support to their patient group using advanced communication skills, education and health promotion to enable them to reach an optimum level of independence and health.

Responsible for providing evidence based practice.

Will have responsibility for leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines working as part of a multi-disciplinary team and developing excellent interdepartmental relationships with excellent communication with a focus on quality improvement

Work within national policy frameworks relevant to the service and trust local guidance.

Review, interpret, act upon diagnostic investigations and point of care testing which may change individual care plans, liaising with appropriate medical staff where necessary.

To maintain accurate appropriate and concise patient records within agreed databases and electronic systems.

Contribute and lead the multi-professional approach in the management of patients / carers within the specialist area in relation to ongoing care needs

Act as a resource for specialist knowledge for clinical staff on wards / departments.

Ensure effective multi- disciplinary communication.

Provide mentorship and preceptorship as required to all staff in the team or students on placement.
