CAMHS LDT Outreach Service - Band 6 Nurse

Due to career progression we are seeking to recruit a RNLD /RN with LD experience, to join our innovative and nationally renowned Outreach service providing assessment, intervention and treatment for childrenand young people with an Intellectual (Learning) Developmental Disabilitypresenting with a wide range of mental health and neurodevelopmental needs andbehaviours that challenge.

We work with children and young people and their families, across Leicester, Leicestershire and Rutland providing care in a variety of settings which include, Rothesay, patient homes, residential facilitates and community settings.

We seek to work positively with our patients and their families undertaking direct work in family homes in order to reduce the effects of behaviours of concern presented and avoid any possible inappropriate inpatient admission and/or fmaily brealdown.

Predominately,the post holder will work within our outreach service butwill also work alongside the wider MDT undertaking Initial assessments, dutyworker reviews and partaking in the CAMHS LD staff on call rota.

Staff development is key within this service, and training opportunities are actively supported and encouraged.

Due to the wide geographical area own transport or suitable alternative is essential. Reliance upon public transport is not suitable for this post.

Main duties of the job

Thepost-holder will undertake assessment and development of PBS management plans and lead and co-ordinate intervention for patients referred into CAMHS LDT Outreach Service.

This includes supporting junior nurses and HCSWs of bands 3 to 5 to undertake observations/assessment and implement plans within a variety of settings.

The post holder will lead on complex clinical cases, requiring day patient admission, possible prolonged direct intervention and in cases where risks presented are deemed to be high

They will take an active role insupporting the Outreach Manager/senior nurse in themaintenance of clinical nursing standards, and assist in the development of the outreach nursing team and any future developments of the service.

The post holder will undertake a robust clinical and line management supervision system that will lead to evidence based care.

The postholder will be required to work shift patterns of early, late and long days, including weekends and bank holidays. Participation in the outreach on call rota for weekends and bank holidays is also required. Flexibility in start and finish times is required when patient / service need dictates
