Main duties of the job

We are looking forenthusiastic, committed, and professionally registered candidates with theclinical expertise and confidence to assess and treat patients with a range ofconditions. To manage a clinical caseload and deal with presenting patient'sneeds in primary care settings as well as providing clinical leadership whereappropriate within the Primary Care Network and support other members of theteam to develop and maintain clinical skills appropriate to your role andexpertise. The successful candidate will have direct contact with patients,taking medical histories, carrying out physical examinations, updating records,making diagnoses, analysing test results, undertaking home visits anddelivering treatment. Working to a professional code of ethics you will be ableto demonstrate flexibility in the role and have a caring attitude towardspatients.

Job description

Job responsibilities

You will manage a caseload of patients.

To screen patients for disease risk factors andearly signs of illness;

To develop with the patient an on-going nursing careplan for health;

You must be able to assess and diagnose patientsboth remotely and in person;

You will discuss patient-related concerns with therelevant staff, and where required, you will be supported to follow appropriatesafeguarding procedures;

You will be competent in triage, in collectingpathological specimens including blood samples, and in monitoring physiologicalobservations using a structured approach along with ordering investigations;

You will administer medication and vaccinationswithin the relevant Patient Group Directions, or, if qualified and appropriate,via Independent Prescribing;

You will continue to develop and maintain youradvanced clinical skills and continuously improve personal specialised skillsthrough audit, research, review and fulfilling training needs.

You will work as part of a diverse team,collaborating with colleagues across the Network to provide the best care toour patients;

You will ensure safe handover of care within andoutside the GP Practices as appropriate.

You will have a key role in the delivery of theNetwork Contract DES specifications, and you will support the coordination anddelivery of Multi-Disciplinary Teams where needed.

You will contribute to the supervision, training,development and coaching of other paramedic staff through peer-support,ensuring a safe and high-quality service is delivered, promoting agreed medicaland emergency activities and development programmes.

To provide paramedic practitioner activities in aprimary care setting to support the GPs, holding clinics and visiting patientswithin their own homes.

To actively pursue policies that will promotehealthy lifestyles for patients: promote health education and participate inrelevant sessions within the practice.

Ensure the clinical computer system is kept up todate with accurate details recorded and amended.
