Job summary

We are recruiting Paramedics to support general practice in Bolton and in particular, the PCN and its objectives. The successful candidate will work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate. The post includes working within the Turton PCN practices and completing home visits to patients both in their own homes and the community setting.

The post will work with all the six GP surgeries known as Primary Care Networks across the Turton PCN.

The post will be based at and provide the service from the GP Surgeries.

The ideal candidate will have experience in working minor illness clinics or have relevant experience in assessing patients and will be registered with the HCPC.

Main duties of the job

Working closely with Primary Care Network Clinical Directors and take direction from them as to the needs of the service. To support the PCN GP practices with home visiting workload.

To work as an autonomous practitioner working independently and with other health care professionals to assess, diagnose and treat patients with a range of acute, non-acute and chronic medical conditions, initiating treatment as appropriate.

The proactively manage housebound/care home patients to increase the quality of care provided to this vulnerable group of patients.

The assess, diagnose and treat undiagnosed and diagnosed conditions for patients within a primary care setting.

To utilise telephone triage for all home visit requests were appropriate.

To collaborate with GPs, Nursing staff and non clinical support staff.

Use their individual skill and experience in order to make a positive contribution to the organisation.

To develop innovate concepts, model, methods and practises which deliver new and improved primary care services to meet to the changing needs of primary care.

To trial and adopt new working methods designed to manage the increasing demand in primary care.

To provide healthy lifestyles, health promotion and disease prevention, in particular to housebound patients.

About us

The post will work with all the six clusters of GP surgeries known as Primary Care Networks across Turton PCN. The post will be based at and provide the service from the GP surgeries.

About us

Turton primary care network, Bolton,consisting of the following practices:

Harwood medical practice

Mandalay medical practice

Oaks medical practice

Dr Liversedge and partners medical practice

Crompton View medical practice

Edgworth medical practice

Turton primary care network was established in 2019 and consists of six practices based geographically together in North Bolton. It is one of nine primary care networks in Bolton. Its total patient population is around 46,000.

The network is responsible for joint working across the network on priorities set by NHS England and Greater Manchester Integrated Care Board Bolton. This includes developing the general practice workforce through the Additional Roles Re-imbursement Scheme (ARRS). The network has a range of health care professionals working in our practices who are either directly employed by Edgworth medical practice on behalf of the network or on secondment from local NHS Trusts. It is anticipated that more health professionals, including this post will be employed directly by the network.

We serve a diverse population including young families and children, working aged people and a high proportion of elderly people. We provide health care to four care homes. The practices are a lovely place to work with friendly and dedicated teams committed to providing the best possible care for their patients. All six practices were rate in their last CQC inspections.

Person Specification

Experience

Essential

Current Registered Paramedic Practitioner (HPC) - . Minor Illness course / qualification. . Evidence of continuing professional development. . Minimum 5 years post registration experience.

Desirable

Completed ACP Course (Degree Level) . Independent Non-Medical Prescriber (V300) . Experience and /or qualification in Mentoring . Relevant / Associated MSc . Benchmarking . Leading a team . Clinical Audit & Research
