Job summary

Do you enjoy making a difference to patient care? This is an exciting opportunity to join Leicester Hospitals in a newly funded Advanced Pharmacist Practitioner Anticoagulation job. Given this is new to the organisation, you will be working closely with key stakeholders across several clinical areas, patient safety and primary careon both trust and system wide initiatives. Key contacts will be our Haematology Consultant Pharmacist, Specialist Medicines Team, Surgical Team, Clinical governance team and Haematology clinical team. An independent prescribing qualification will be required for the role, or a commitment to work towards one. Previous specialist experience of anticoagulation will be of benefit but is not essential as training will be provided. Key to this role is collaborative working and excellent communication skills, good insight into clinical governance related to anticoagulation and pragmatic solutions to complex problems. In this role we will support you with dedicated time to train, leadership development opportunities, independent prescribing training and regular opportunities for innovation and improvement. This role is an 8b Advanced Pharmacist Practitioner post. There is also the opportunity for you to apply to join this role as an 8a and undertake a development plan with the aim of developing you to gain the skills and knowledge to progress to an 8b. The role will have a bespoke job plan.

Main duties of the job

The post holder will be responsible for the provision of a highly specialised pharmacy service to patients across the Trust.

They will be expected to provide expertise to nursing and medical staff (within the Trust and externally) on the management of individual patients, ensuring the safe and efficient provision of anticoagulation.

They will be expected to integrate with the relevant clinical teams to ensure efficient transfer of clinical and medicines management issues.

They will take overarching responsibility for all pharmacy research and audit projects within the service and be responsible for overseeing training including undergraduate placements, trainee pharmacists and diploma including independent prescribing.

Registered with GPhC Member of professional organisation RPS, NPPG or UKCPA Registered practicing Independent Prescriber

Significant experience at band 8a or equivalent in relevant speciality Significant experience of delivering education and training at post-graduate level Portfolio of evidence showing achievements in practice/speciality Broad knowledge on the use of medicines used within speciality Evidence of presentations at local, regional, national level.
